Foreign-Owned LLC Filing (Form 5472)
Fax number: (855) 887-7737. Send at 300 DPI minimum. Required annually for all foreign-owned US single-member LLCs. Cannot be e-filed for disregarded entities. Penalty for non-filing: $25,000 per form. Due April 15 (extensions available via Form 7004).
S-Corp Election (Form 2553)
Eastern states: (855) 887-7734. Western states: (855) 214-7520. Cannot be e-filed. Deadline: 2 months and 15 days after tax year start. IRS responds with CP 261 (accepted) or CP 264 (denied) by mail in approximately 60 days.
Power of Attorney (Form 2848) and Tax Info Auth (Form 8821)

Does the IRS confirm receipt of faxed forms?

Can I fax my tax return (1040, 1120) to the IRS?
No. Individual and corporate tax returns cannot be faxed. They must be e-filed or mailed. The IRS only accepts specific forms by fax (listed on this page).
What time should I fax to the IRS?
Most IRS fax lines are available 24/7 (including SS-4 Fax-TIN). If the line is busy, try early morning Eastern time or weekends. Use high quality (300 DPI) for best results.
Is a fax confirmation legal proof of timely filing?
A fax confirmation is evidence of delivery, but it does not get the legal protection of IRC Section 7502 ("timely mailing = timely filing").
