What goes on a fax cover sheet
The first page of a fax. It tells whoever picks it up who it is for, who sent it, how to reply, and how many pages to expect.

How to fill out a fax cover sheet

Write the recipient's name and fax number
Check the number against their own listing. A fax to a wrong number cannot be recalled.

Add your name and a way to reach you
Your fax number if you have one; otherwise an email address, or a phone number in the message.

Count the pages, including the cover
The recipient uses the count to know whether every page arrived.

Say what is attached

What do you write on a fax cover sheet?

Does the cover sheet count as a page?
Yes. It is a page of the fax like any other, so count it in the total. Sent from FaxTerra, an optional cover page adds one page to the count.
Is a fax cover sheet required?
Most recipients do not require one, and FaxTerra's cover page is optional. Two cases where you want one: a shared machine, where the cover routes the fax to the right person, and sensitive documents, where the cover carries the confidentiality notice. Some courts and agencies have their own cover form; when they do, use theirs as page one.
Do I need a cover sheet to fax the IRS?
The IRS acts on the form, not the cover. A cover helps for Form SS-4 because the IRS faxes the EIN back and the cover repeats your return fax number. The SS-4 cover sheet on this site has the IRS EIN Operation and the right line already on it.
Can I fax without a cover sheet?

Is a fax cover letter the same thing?
Same page, two names. Some people write a longer message on it, which is where "letter" comes from. The fields are the same.
