Form 3911 Fax Numbers by State
Form 3911 goes to one of eight Refund Inquiry Units, and the state you live in decides which one. Every unit takes it by fax or mail.

Before you send it
The IRS says you should receive a refund check "within 4 weeks from the date we issue it," so confirm that time has passed first. The issue date is in the IRS refund tracker or your online account.
Form 3911 is the taxpayer statement the IRS uses to trace a refund that was issued and never arrived. A filer whose status is single, married filing separately or head of household can start a trace on a current-year paper check in the refund tracker instead. A joint filer calls the IRS or sends Form 3911.
On the form:
- Complete a separate Form 3911 for each refund.
- Fill in every field, and mark any part that does not apply as N/A.
- If the refund was from a joint return, both spouses must sign before the IRS can begin a trace.

How to send it
The IRS page says: "Do not send anything other than a Form 3911 to the fax numbers below." Save the signed form as a PDF or photograph it, upload only that, enter the fax number for your unit, and send. The IRS page for Form 3911 describes no acknowledgment. Keep your confirmation with the date you sent the form. It is a timestamped record that the IRS fax line took every page, and the proof-of-filing guide covers what it does not show.
