The Form 941 instructions encourage electronic filing and give a mailing address for a paper return. No fax number appears for the return itself.
Mailing addresses by state
The IRS encourages you to file Form 941 electronically, and its instructions point to the IRS employment e-file page for the ways to do it. A paper return is mailed, and where depends on whether a payment is enclosed.
Without a payment, to Department of the Treasury, Internal Revenue Service:
- Kansas City, MO 64999-0005: CT, DE, DC, GA, IL, IN, KY, ME, MD, MA, MI, NH, NJ, NY, NC, OH, PA, RI, SC, TN, VT, VA, WV and WI.
- Ogden, UT 84201-0005: AL, AK, AZ, AR, CA, CO, FL, HI, ID, IA, KS, LA, MN, MS, MO, MT, NE, NV, NM, ND, OK, OR, SD, TX, UT, WA and WY, and exempt organizations, governmental entities and Indian tribal governmental entities, regardless of location.

Which payroll forms can I fax to the IRS?
A new employer can apply for an EIN by faxing Form SS-4. An employer that needs more time to furnish W-2s or 1099s to recipients can fax Form 15397, covered in the Form 8809 guide. An accountant is authorized to act for you with Form 2848, or to see your records with Form 8821.
