Form 9465 Fax Number
The IRS lists no fax number for Form 9465. The instructions give two routes: mail, or the online payment agreement if you owe $50,000 or less.
Mailing Form 9465
Filed with a return, Form 9465 is attached to the front of the return and mailed with it, to the address in the return instructions. Filed on its own, after the return or in reply to a notice, it is mailed to the service center for your state in the Where To File table of the Form 9465 instructions.
That table has three parts:
- One address list for most filers.
- One for filers with a Schedule C, E or F.
- Department of the Treasury, Internal Revenue Service, 3651 South I-H 35 5501AUSC Austin, TX 78741, for filers in a foreign country or U.S. territory, filers with an APO or FPO address, Form 2555 or 4563 filers, and dual-status aliens.
Bona fide residents of American Samoa, Puerto Rico, Guam, the U.S. Virgin Islands or the Northern Mariana Islands should see Pub. 570.
Applying online
If you owe $50,000 or less in combined tax, penalties, and interest, you can apply for a payment plan at IRS.gov/OPA instead of filing Form 9465. The instructions say the user fee is lower that way.
What happens next
The IRS says it will "usually let you know within 30 days after we receive your request whether it is approved or denied," and that it can take longer for tax due on a return you filed after March 31. If the IRS sends you Form 433-D to sign, the Form 433-D guide covers where it goes.
