No Colorado agency below publishes a general fax line for its program's documents: 2 run their programs through local offices, so the number is your own county's. Each row says what its line takes.

The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.

Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.

The Member Contact Center asks for the member's Health First Colorado ID number, date of birth, and the last four digits of the member's Social Security number, which are the identifiers a county office also works a case from.

The Colorado Department of Human Services says a paper SNAP application is mailed, faxed or returned in person to the applicant's own county human services office, so the fax number is the county's and not a state one.

The Colorado Division of Unemployment Insurance asks that the barcoded CDLE form go on top of any supporting documents and that the claimant's name and Claimant ID appear on every page that is not a barcoded form, and that each form go to the fax number printed on it.

What do I put on every page I fax to a Colorado agency?

Your case or client number, your name, and your date of birth, unless the row below says the office asks for something else. A page that arrives without a case number has to be matched by hand, which is the slowest path through any agency.

My row says the program runs through county offices. Where do I fax it?

To your own county office, whose fax is in the state directory linked in that row. Colorado does not run that program from one address, so there is no statewide number to give you, and a number from another county will not reach your caseworker.
