Where to fax it

The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.
Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.
The New York State Department of Health says a Medicaid applicant is told which proofs are needed, of residency, citizenship or immigration status, household size and income, before the application can be processed.
The New York State Department of Labor asks for the claimant's Social Security number at the top right corner of every page faxed or mailed about an unemployment claim, and says to keep the fax confirmation sheet because it does not confirm receipt.
The New York State Workers' Compensation Board, which runs statutory disability benefits and Paid Family Leave, states that it accepts claim-related forms and documents only by email, web upload and mail, not by fax.
The Workers' Compensation Board asks for case-identifying information, the WCB Case Number, on the upper right of every page, or the claimant's name and date of accident where no case number has been assigned yet, and says a document may not reference more than one claimant.
The New York State Department of Taxation and Finance states that it has no central fax number.

What do I put on every page I fax to a New York agency?

My row says the program runs through county offices. Where do I fax it?
To your own county office, whose fax is in the state directory linked in that row. New York does not run that program from one address, so there is no statewide number to give you, and a number from another county will not reach your caseworker.
One of these offices takes no fax at all. Is that right?
Yes, and that row quotes the agency saying so. Faxing a document to an office that does not accept faxes loses the document with no record that you sent it, so use the channel the row names instead.
