Fax Your Ohio Benefits Paperwork

The fax line each Ohio agency publishes for Medicaid and CHIP, SNAP, unemployment, state leave benefits, and state tax, with what that office asks you to put on every page.

Where to fax it

No Ohio agency below publishes a general fax line for its program's documents: 2 run their programs through local offices, so the number is your own county's, and 2 take no fax and say where to send instead. Each row says what its line takes.

The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.

Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.

Medicaid and CHIP
County departments of job and family services

Ohio says a county job and family services agency decides medical assistance within 45 days of the date the application was turned in, and longer where a disability determination is needed.

SNAP and Ohio Works First
County departments of job and family services

Ohio tells an applicant to fax SNAP and cash assistance documents to their own county job and family services agency, naming that agency's own fax number as the destination rather than a state line.

Ohio may approve SNAP and cash assistance back to the date the application was turned in.

The Ohio Department of Job and Family Services publishes no fax for an unemployment claim: it takes the information a claim needs through the account a worker logs into.

The Ohio Department of Taxation gives a PO Box and no fax for an individual income tax billing or assessment notice reply, and prints a fax only beside its business tax addresses.

Questions

What do I put on every page I fax to a Ohio agency?

Your case or client number, your name, and your date of birth, unless the row below says the office asks for something else. A page that arrives without a case number has to be matched by hand, which is the slowest path through any agency.

Can I fax a Ohio agency without a fax machine?

Yes. Upload the pages from your phone or computer, enter the office's fax number, and send. The agency receives it as a fax. Every account gets 10 free pages a month, and a page to a US fax number counts as one.

What if the fax line does not answer?

Call the number in that office's row before you try again. A line that rings and never connects will not start working on a retry, and the office can tell you whether the number changed or whether it wants the document another way.

My row says the program runs through county offices. Where do I fax it?

To your own county office, whose fax is in the state directory linked in that row. Ohio does not run that program from one address, so there is no statewide number to give you, and a number from another county will not reach your caseworker.

One of these offices takes no fax at all. Is that right?

Yes, and that row quotes the agency saying so. Faxing a document to an office that does not accept faxes loses the document with no record that you sent it, so use the channel the row names instead.

Is this an official Ohio site?

No. FaxTerra is a fax service. Ohio and the office you are writing to set the rules and decide your case; every number and requirement here is linked to the agency's own page under official sources.
