Banner Health Fax Number for Medical Records

Banner publishes one fax, and it is not the patient route

Banner Health handles release of information through a vendor partner, MRO, and splits the traffic in two.

A hospital or provider's office sending a records request faxes it to (408) 608-2607. Third-party requesters use the same number: Banner names payers, including Risk Adjustment and HEDIS requests, attorneys, and insurance companies. Banner also states that an urgent continuation-of-care request sent after hours can still go to that number to be processed.

A patient asking for their own record is directed somewhere else. Banner lists three routes: its patient portal for the health information summary, an online request through "MRO Patient Central, a partner of Banner Health", or a visit to the Health Information Management Services department at any Banner hospital with a government-issued photo ID. No patient-facing fax number is published, so this page does not print one.

Which route fits what you are doing

The split is about who is asking, not what is being asked for:

• You are a patient wanting your own chart. Use the online request or the portal. If you are a parent of a minor or hold power of attorney for an adult patient, expect to upload documentation proving you are authorized.
• You work at a clinic and need records for continuing care. That is the fax line above, and it is the fastest of Banner's routes.
• You are an attorney, insurer, or payer. Same fax line, and Banner points third-party requesters at MRO's national service center for status.
• You need a Banner physician's completed disability or leave paperwork faxed onward. That is a request to the physician's office, not to release of information.

Banner covers hospitals in Arizona, Colorado, and Wyoming, plus Churchill Community Hospital in Nevada, Lassen Medical Center in California, and Ogallala Community Hospital in Nebraska. The same routing applies across all of them.

Verify the number before you send

A records request carries a patient's name, date of birth, and treatment dates, and it is going to a vendor rather than to Banner directly.

• Read the number off bannerhealth.com's Other Medical Records page, which is where Banner publishes it. Do not take it from a search snippet.
• If Banner or MRO wrote to you asking for something specific, use the number in that letter.
• A patient request faxed to the provider line is not a shortcut. It goes to a queue built for provider traffic and will be slower than the online route Banner points patients at, if it is worked at all.
• Banner notes a fee may apply for labor, supplies, and postage, and suggests requesting a specific document rather than the full record to keep the cost down.
