Baylor Scott & White Fax Numbers for Medical Records

Baylor Scott & White publishes one fax number for the whole system, handled by its records partner, plus a Health Information Management line at each of its hospitals. Either works. The system line is the simple choice; the hospital line puts the request in front of the department that holds your chart. Every number below is read from Baylor Scott & White's own records page.

One system line, plus a line at each hospital

Baylor Scott & White gives patients a choice most large systems do not. A completed Authorization for Release of Information can go to a single system-wide fax at (855) 563-2794, handled by HealthMark Group in Addison, Texas, and that route covers every hospital and clinic in the system. Baylor Scott & White writes the number as 855.563.BSWH (2794) on its own page; dial the digits.

Each hospital also publishes its own Health Information Management fax, in a "Select a location" panel on the same page with the office's street address and phone. Every one of those panels is in the table above, grouped by region: the Dallas-Fort Worth hospitals and Heart Hospitals, the Central Texas hospitals around Temple and Waco, and the Austin-area hospitals. Baylor University Medical Center in Dallas uses (214) 818-6782 and the Temple medical center uses (254) 724-0676, which also serves the Continuing Care Hospital.

Two splits to read carefully. The Heart Hospitals are separate from the medical centers that share their city: Plano, McKinney and Waxahachie each have a medical center row and a Heart Hospital row, and the Dallas Heart Hospital records office also serves the Fort Worth and Waxahachie Heart Hospital locations. And clinic records are the one real fork: a visit to a HealthTexas Provider Network clinic is a physician practice record, not a hospital record, and goes to (800) 833-5935, also handled by HealthMark Group. If your care was a hospital stay and a clinic follow-up, that is two requests.

What Baylor Scott & White says about the request

Baylor Scott & White publishes no restriction on faxing. Its page describes the system route as the way to submit a completed request form for all medical records, by email, fax or mail, and introduces the facility list as the way to contact a specific Health Information Management department. It also publishes a separate phone line for questions about a request and for checking its status, which is the route to use once the fax has been delivered.

Records can come back to you through the MyBSWHealth app, by mail, or released directly to your personal email address; pick one on the form.

What you are likely sending

Baylor Scott & White publishes four versions of the authorization form, in English and Spanish, in two directions:

  • Authorization for release of information from Baylor Scott & White, to get your records out to yourself or another provider
  • Authorization for release of information to Baylor Scott & White, to bring outside records into your chart
  • A signed request naming the dates of service and the record types you want
  • Disability, FMLA, or leave paperwork a Baylor Scott & White physician has completed

An authorization plus an ID copy is two or three pages, inside the free monthly allowance on every account.

FAQ

Frequently Asked Questions

What is the Baylor Scott & White fax number for medical records?

Baylor Scott & White publishes (855) 563-2794 as a system-wide fax for completed authorization forms, handled by HealthMark Group in Addison, Texas. Each hospital also publishes its own Health Information Management fax: Baylor University Medical Center in Dallas is (214) 818-6782, Temple is (254) 724-0676, All Saints in Fort Worth is (817) 922-7052, and the other twenty-plus hospitals and Heart Hospitals are in the table above. Clinic records go to a separate HealthTexas line. Every number is read from Baylor Scott & White's own records page.

Should I use the system fax or my hospital's fax?

The system line covers every facility and is the simpler choice when you are not sure which hospital holds the record. Use the hospital's own line when you already know the facility and want the request in front of that department directly.

How do I get records from a Baylor Scott & White clinic rather than a hospital?

Clinic visits are HealthTexas Provider Network records and go to (800) 833-5935. A hospital stay and a clinic visit produce two separate records and need two separate requests.

Which line do I use for a Heart Hospital?

Each Heart Hospital has its own row in the table, separate from the medical center in the same city. The Dallas Heart Hospital records office also serves the Fort Worth and Waxahachie Heart Hospital locations, per Baylor Scott & White's own page, while Denton, McKinney and Plano each have their own line.

Can I get my Baylor Scott & White records without a form?

Records already released to your MyBSWHealth account are available there with no form. Anything beyond that needs the signed authorization, which is where the fax number comes in.

Is FaxTerra affiliated with Baylor Scott & White?

No. FaxTerra is an independent online fax service. We deliver your document to the number you enter and confirm when it arrives. We are not affiliated with Baylor Scott & White Health and cannot look up your records.
