One system line, plus a line at each hospital
Baylor Scott & White gives patients a choice most large systems do not. A completed Authorization for Release of Information can go to a single system-wide fax at (855) 563-2794, handled by HealthMark Group in Addison, Texas, and that route covers every hospital and clinic in the system. Each hospital also publishes its own Health Information Management fax, in a "Select a location" panel on the same page with the office's street address and phone. Every one of those panels is in the table above, grouped by region: the Dallas-Fort Worth hospitals and Heart Hospitals, the Central Texas hospitals around Temple and Waco, and the Austin-area hospitals.
