Cigna Prior Authorization Fax Number
Cigna lists one fax line for medical precertification and points medication requests to ePA portals or the phone. Its medication form still prints a fax of its own. The provider’s office sends these requests, and the line depends on who handles them.
HIPAA-compliant faxing from FaxTerra, $9.98 a month
Starter is $9.98 a month with 50 pages. Business is $14.98 with 200 pages, Pro $34.98 with 1,000. Each of these plans includes a signed Business Associate Agreement, encryption of every document, and a six-year audit log. Unlimited additional users can be added.
Which line takes which request
Cigna’s precertification page separates who handles the request:
- A service Cigna Healthcare handles directly. Send the precertification intake form to fax (866) 873-8279, call (800) 882-4462, or use an EDI 278 transaction from your practice system.
- A service EviCore by Evernorth manages. Cigna routes these to EviCore’s phone and portal. Of the EviCore services on the page, only musculoskeletal pain management, joint surgery and spinal procedures show a fax, (800) 540-2406.
- A medication. Cigna’s page names three ePA routes (EviCore, Surescripts and CoverMyMeds) and says to call (800) 882-4462 if you cannot use one. It names no fax line for medication.
Check the Master Precertification List on the same page to see whether the service needs review at all.
The medication form and its fax line
Cigna’s Medication Prior Authorization Form, revision v102015, tells you to “Fax completed form to: (855) 840-1678” and to call (800) 882-4462 for an urgent request. The Arkansas version of the form prints the same line. Cigna’s current precertification page does not mention it, and leads with ePA or the phone.
The form also says Cigna will not respond via fax with the outcome unless every asterisked field is complete. Those fields are the physician name, the DEA, NPI or TIN, and the patient’s name, Cigna ID, date of birth and street address.
What to send
The medical form asks for the patient and Cigna ID, the requesting and servicing providers with their Tax ID or NPI, the date of service, the diagnosis and procedure codes, and supporting clinical notes. Cigna’s page warns that a request missing important details may be denied.
The medication form asks for the drug, strength and dosing schedule, the ICD-10 code, and each alternative tried with dates and results. If you add information after the first request, Cigna asks: “please use the same method you used before”.
Match the form to the handler
Cigna, EviCore and Cigna’s pharmacy forms each have their own line, and a request sent to the wrong one starts over.
- Check whether Cigna or EviCore handles the service before you pick a number.
- Use the number on Cigna’s denial or request letter when one names a destination.
- Keep the delivery confirmation. It shows the fax reached the number, not that Cigna has decided the request.
Where Cigna’s answer arrives
Both Cigna forms ask for the office fax, and the medication form says Cigna answers by fax once its required fields are complete. A FaxTerra paid plan includes a dedicated number, and what arrives lands in the same inbox the practice sends from, on the web or the mobile app.
For PHI, a covered practice adds HIPAA compliance, $4.99 a month on top of any paid plan, which includes a signed Business Associate Agreement. Plans without it, including the free one, have no BAA.

Frequently Asked Questions
What is the Cigna prior authorization fax number?
For medical precertification handled by Cigna Healthcare, (866) 873-8279. For medication, Cigna’s page points to ePA or the phone at (800) 882-4462, and its Medication Prior Authorization Form prints (855) 840-1678.
Is (855) 840-1678 still Cigna’s medication fax?
Yes, on the form. It is the number printed on Cigna’s Medication Prior Authorization Form, revision v102015, which Cigna still serves. Cigna’s precertification page does not list it and leads with ePA, so a call to (800) 882-4462 can confirm it before you send PHI.
Where do Cigna referrals go by fax?
To the same line. Cigna’s page tells a primary care provider to find the form in the Forms Center and fax it to (866) 873-8279. A specialist can confirm a referral by phone.
Is this the fax for my own Cigna records?
No. A member’s request for their own records or an authorization to release them goes to the Central HIPAA Unit, covered in the Cigna guide. A patient sending their own paperwork needs no BAA and can use the free plan.
Is FaxTerra affiliated with Cigna?
No. FaxTerra is an independent online fax service. We deliver your document to the number you enter and confirm when it arrives. We cannot see a request’s status or decide coverage.
