Which Cigna request goes by fax
Cigna routes health-information requests through one team, the Central HIPAA Unit, and prints both of its fax lines on the relevant forms: (877) 815-4827 and (859) 410-2419, with mail going to Cigna Healthcare Central HIPAA Unit, PO Box 188014, Chattanooga, TN 37422. Two forms use it:
- The Authorization for Disclosure of Protected Health Information, which lets a spouse, attorney, or other person see your information
- The Request for Access to Protected Health Information, which gets you a copy of what Cigna holds, including treatment and diagnosis codes
Cigna’s Customer Appeal Request form is different: it is mail-only, and the address is on the form and on your Explanation of Benefits. A coverage appeal is not a HIPAA Unit matter.

Both forms turn on identifying the right person and the right records:
- Your name, date of birth, and Cigna member ID
- Who is authorized, if you are releasing information to someone else, with their name and address
- The specific records and the date range you want covered
- A signature from the customer, personal representative, or parent or guardian
Cigna limits what a subscriber or legal representative can sign for on a minor’s behalf for certain sensitive services, so read the form’s notes if the request concerns a minor. A signed form is one to two pages, which sends free.
