Corewell Health routes records by hospital, and lists every line

Corewell Health is the system formed from Beaumont Health in southeast Michigan and Spectrum Health in west Michigan, and it kept the hospital-level records offices from both. Its Request a Copy page has a collapsed panel per hospital, each with a Health Information Management department, its address, phone and fax. There is no system-wide number. The table above is read from those panels, one row per fax, grouped into southeast Michigan, the Lakeland hospitals in southwest Michigan, and west Michigan.

Pick the row for the hospital that treated you. Corewell Health William Beaumont University Hospital in Royal Oak uses (248) 898-7432, the Grand Rapids hospitals use (616) 391-1521, and the Lakeland hospitals share (269) 982-4305. Every other hospital has its own line in the table. If your care spanned two Corewell hospitals, that is two requests.

The splits worth reading twice are the hospitals with no records staff on site, which Corewell states in the panels themselves. Wayne Hospital shares the Trenton line, and pickup for Wayne records is at Dearborn, Trenton or Taylor. Blodgett, Helen DeVos Children's and the Meijer Heart Center share the Grand Rapids line, and pickup is at Butterworth. Reed City shares the Big Rapids line, and pickup is at Big Rapids. Imaging is separate from the chart: the Lakeland hospitals and the west Michigan hospitals each publish an imaging-request fax, listed as their own rows, while the southeast Michigan hospitals take imaging requests by phone and email only. The two Rehabilitation and Nursing Center lines cover patients discharged through May 17, 2026.

What Corewell says about the form

Corewell's page states one requirement on the form itself: "Please print, complete, and hand sign this authorization form, then send it to the requested location listed below." So print the authorization, sign it by hand, and fax the signed copy; a typed name in the signature box is the reason a request comes back. For images, Corewell notes that copies shared electronically or sent directly to a health care provider carry no charge, while a CD carries a small fee, so name the receiving provider on the imaging request if that is where the images are going.

What you are likely sending

The requests that need a signature and a destination:

• A signed authorization releasing your chart to yourself, a new provider, or an attorney
• A records request when you are moving out of the Corewell system
• An imaging request, which goes to the imaging line for your region rather than the chart line
• Disability, FMLA, or leave paperwork a Corewell physician has completed and signed
• Outside records going the other way, so a Corewell clinic has your prior history

Birth certificates are not a Corewell records request. Those go to the county clerk for the county the hospital sits in, and Corewell lists the county office next to each hospital for that reason.
