Corewell Health has no single records fax
Corewell Health is the system formed from Beaumont Health in southeast Michigan and Spectrum Health in west Michigan, and it kept the hospital-level records offices from both. Its Request a Copy page lists a Health Information Management department for each hospital, with its own address, phone, and fax. There is no system-wide number.
The three numbers above are the largest sites in each region. The rest of the network has its own: Dearborn, Farmington Hills, Grosse Pointe, Taylor, Trenton, Troy, and Wayne in southeast Michigan, and Big Rapids, Gerber, Greenville, Ludington, Pennock, Reed City, and Zeeland in west Michigan. Several share a number with a nearby site, so guessing between them is not safe.
The hospital that treated you is the one holding the chart. If your care spanned two Corewell hospitals, that is two requests.
What Corewell patients send by fax
The requests that need a signature and a destination:
- A signed authorization releasing your chart to yourself, a new provider, or an attorney
- A records request when you are moving out of the Corewell system
- Imaging records, which the Grand Rapids office handles for the hospitals that do not do their own pickup
- Disability, FMLA, or leave paperwork a Corewell physician has completed and signed
- Outside records going the other way, so a Corewell clinic has your prior history

- Skip third-party directories. Several still list the pre-merger Beaumont and Spectrum numbers.
