Dignity Health routes by facility, not by system

Dignity Health publishes a Health Information Management contact for each hospital, and its instruction is the same everywhere: complete the Authorization to Disclose form, then submit it by mail, fax, or in person at one of the medical records departments. There is no system-wide records fax.

The four numbers above cover the largest sites in Arizona, greater Sacramento, Southern California, and Nevada. Greater Sacramento is the one region that consolidates: a single Rancho Cordova office takes requests for every Sacramento-area facility, and the form asks you to name which one. Everywhere else the number belongs to a specific hospital.

Dignity Health is part of CommonSpirit Health, and its pages now carry the CommonSpirit name while the Dignity Health brand and the dignityhealth.org site remain live. Records requests are unaffected by that, but it explains why a Dignity Health form can route to a commonspirit.org email address.

What Dignity Health patients send by fax

The requests that need a signed form and a destination:

• An Authorization to Disclose, releasing your records to yourself, a new provider, an employer, or an attorney
• A Patient Request for Access, when you want your own copy rather than a release to someone else
• Billing records, which the same authorization covers when you tick the billing option
• Disability, FMLA, or leave paperwork a Dignity Health physician has completed and signed
• Outside records going the other way, so a Dignity Health clinic has your prior history

Dignity Health asks you to state clearly who can receive your records and for what purpose. Vague answers are the most common reason a records office calls back before doing anything. A signed form plus a photo ID copy is two to three pages, which sends free.

Verify the number before you send

A Dignity Health authorization names you, your date of birth, the facility, and the records being released.

• Find your hospital's page on dignityhealth.org and read the fax number in the medical records section. Each facility publishes its own, with the address and the office hours.
• If your care was in greater Sacramento, use the regional number and write the specific facility on the form.
• If a Dignity Health department wrote to you, use the number in that letter.
• Skip third-party directories. Several list numbers that are close to the published ones but not identical, which is the failure that goes unnoticed.
