Dignity Health Fax Numbers for Medical Records

Dignity Health routes by facility, not by system

Dignity Health is part of CommonSpirit Health, and its medical records pages now live on commonspirit.org: a dignityhealth.org records address redirects to a CommonSpirit hub with a picker for nine Dignity Health regions, and each regional page lists a Health Information Management block per hospital, with the address, the phone number and the fax. The numbers above are read from those nine pages. There is no system-wide records fax.

Pick the row for the hospital that treated you. The instruction is the same on every regional page: "You can submit the form by mail, fax or in person at one of our medical records departments." A signed authorization sent to the wrong hospital's office sits with a department that has no chart to release. A few regions consolidate or split in ways worth knowing. Greater Sacramento runs one Rancho Cordova office for every Sacramento-area facility, and the form asks you to name which one. Marian Regional and Arroyo Grande share one office in Santa Maria, and St. Joseph's Westgate uses the St. Joseph's Phoenix office. Clinic records from a Dignity Health Medical Group practice in Arizona or Nevada go to the medical group's own line rather than to a hospital, and Pacific Central Coast Health Centers clinic records go to an outside records vendor.

A few sites publish no fax at all. Yavapai Regional Medical Center in Arizona routes requests through an online request portal. Mercy Medical Center Merced and St. Joseph's Behavioral Health Center in Stockton publish mail addresses only, and the medical group clinics outside Arizona and Nevada publish phone numbers only. For those, the regional page has the route that does work.

What Dignity Health says about the Rose de Lima campus

The Nevada page carries one restriction worth quoting. The records department at St. Rose Dominican, Rose de Lima Campus "is not available for in-person assistance and only offers assistance via mail or email requests", and the page sends medical and billing record requests to the Siena or San Martin campus instead. So a Rose de Lima chart is requested by mail or email, or in person at one of the other two St. Rose campuses; there is no Rose de Lima fax to send to.

Everywhere else the fax is a submission route for the signed form. Dignity Health asks you to state clearly who can receive your records and for what purpose. Vague answers are the most common reason a records office calls back before doing anything.

What you are likely sending

• An Authorization to Disclose, releasing your records to yourself, a new provider, an employer, or an attorney
• A Patient Request for Access, when you want your own copy rather than a release to someone else
• Billing records, which the same authorization covers when you tick the billing option
• Disability, FMLA, or leave paperwork a Dignity Health physician has completed and signed
• Outside records going the other way, so a Dignity Health clinic has your prior history

A Dignity Health authorization names you, your date of birth, the facility, and the records being released. A signed form plus a photo ID copy is two to three pages, which sends free. If a Dignity Health department wrote to you, use the number in that letter. Skip third-party directories: several list numbers that are close to the published ones but not identical, which is the failure that goes unnoticed.
