Fax a Medicare Appeal or Enrollment Form
The fax-able Medicare tasks — appeals, redeterminations, and plan enrollment — sent to the right contractor or plan, with a number so their decision reaches you.

Match the task to the right destination
Medicare is not one fax line. A level-1 appeal (redetermination) goes to your Medicare Administrative Contractor (MAC); a Medicare Advantage or Part D matter goes to your plan; an enrollment or disenrollment form goes to the address or fax on that form. The correct destination is printed on your Medicare Summary Notice, denial, or plan paperwork. (Medical *claims* are electronic-only — fax is for the appeal and enrollment paperwork.)
Include your Medicare number and the claim/appeal ID
On the cover sheet, put your Medicare Beneficiary Identifier (MBI) and the claim or appeal ID from your notice, so it routes to your record. For a redetermination, include the completed request (or a written appeal) plus any supporting documentation you are relying on.
Send free — larger packets a few cents a page
Upload your form or appeal (PDF or photos) and enter the MAC or plan fax number. Redetermination requests, enrollment and disenrollment forms, and appeal evidence send free within your monthly free pages. Track delivery so you have proof it arrived before the appeal deadline. Get your own number for the decision
Medicare appeals move on strict deadlines, and a redetermination decision or a request for more information comes back by mail and fax. A dedicated FaxTerra number makes sure that reply reaches you in time — in your email as a PDF. See how receiving works.
Frequently Asked Questions
Can I fax a Medicare appeal?
Yes. A level-1 Medicare appeal (redetermination) can be faxed to your Medicare Administrative Contractor (MAC) — the fax number is on your Medicare Summary Notice or denial. FaxTerra sends it online with delivery confirmation.
What is the Medicare fax number?
There is no single Medicare fax number. Redeterminations go to your MAC; Medicare Advantage and Part D matters go to your plan; enrollment forms list their own fax. Use the number printed on your notice or form.
Can I fax Medicare medical claims?
No — Medicare medical claims are submitted electronically. Fax is for the appeal (redetermination), enrollment, and plan paperwork, which is exactly what this page covers.
Is FaxTerra affiliated with Medicare or CMS?
No. FaxTerra is an independent online fax service, not affiliated with Medicare or CMS. We deliver your appeal or enrollment paperwork to the fax number you provide and receive the reply for you. Enrollment and appeal paperwork only — we are not a HIPAA-covered service.
