There is no single Medicare fax number
Medicare is not one fax line. A level-1 appeal (redetermination) goes to your Medicare Administrative Contractor (MAC); a Medicare Advantage or Part D matter goes to your plan; an enrollment or disenrollment form goes to the address or fax on that form. The correct destination is printed on your Medicare Summary Notice, denial, or plan paperwork. (Medical *claims* are electronic-only; fax is for the appeal and enrollment paperwork.)

Medicaid is run by your state, so its fax number is too
Medicaid has no national fax number either, and for a different reason: it is administered by each state, and in most states delivered through managed-care plans under contract to the state. Your case belongs to the state agency or the plan named on your notice, not to a federal office.
To find the right destination:
- Use the fax number on the Notice of Action or denial letter you received. An appeal form is usually attached to it.
- Otherwise use your plan’s member portal or the number on the back of your Medicaid card.
- A state fair hearing goes to the state agency rather than to the plan, and the notice says where.

Include your Medicare number and the claim/appeal ID
On the cover sheet, put your Medicare Beneficiary Identifier (MBI) and the claim or appeal ID from your notice, so it routes to your record. For a redetermination, include the completed request (or a written appeal) plus any supporting documentation you are relying on.
