Where to fax it
One of the 5 California agencies below publishes a fax line for its documents, 2 run their programs through local offices, so the number is your own county's, one takes no fax and says where to send instead, and one publishes only a line for a particular form or an appeal. Call the number in the row before you send.
The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.
Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.
The California Department of Social Services says a CalFresh household must report and provide verification of certain household information when it applies and at each report, so the county office is expecting the documents its notice lists.
The Employment Development Department asks for the completed Unemployment Insurance Application and every applicable attachment in one submission, and says an incomplete application may delay or prevent the filing of the claim.

The Employment Development Department publishes no fax line for Disability Insurance or Paid Family Leave documents, and tells senders not to use its one public fax line, the WOTC Center's, for them.
A California Disability Insurance claim is complete only when the Employment Development Department has both Part A, the claimant's statement, and Part B, the licensed health professional's certificate, of the DE 2501 form.
The Franchise Tax Board asks for the documents its notice lists and gives 30 days from the date on the notice, so the fax should carry the notice and every document named on it.

What do I put on every page I fax to a California agency?
Your case or client number, your name, and your date of birth, unless the row below says the office asks for something else. A page that arrives without a case number has to be matched by hand, which is the slowest path through any agency.
Can I fax a California agency without a fax machine?

What if the fax line does not answer?
Call the number in that office's row before you try again. A line that rings and never connects will not start working on a retry, and the office can tell you whether the number changed or whether it wants the document another way.
My row says the program runs through county offices. Where do I fax it?
To your own county office, whose fax is in the state directory linked in that row. California does not run that program from one address, so there is no statewide number to give you, and a number from another county will not reach your caseworker.
One of these offices takes no fax at all. Is that right?
Yes, and that row quotes the agency saying so. Faxing a document to an office that does not accept faxes loses the document with no record that you sent it, so use the channel the row names instead.
