The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.
Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.
The Oregon Health Plan application tells an applicant to fax documents behind the yellow coversheet that comes in the application packet, and to fax both sides of each page.
The Oregon Department of Human Services lists per program what it asks an applicant to send: income for each person and identity of the head of household for SNAP, and medical or court ordered child support expenses.
The Oregon Employment Department requires a Social Security Number on the unemployment insurance application so it can verify identity, and says an unanswered question can delay the claim or send the application back.
Paid Leave Oregon states on its contact page that it has no fax number, and asks for documents through a Frances Online account or its Contact Us form instead.

What do I put on every page I fax to a Oregon agency?
