What do I put on every page I fax to a Maryland agency?

To your own county office, whose fax is in the state directory linked in that row. Maryland does not run that program from one address, so there is no statewide number to give you, and a number from another county will not reach your caseworker.

One of these offices takes no fax at all. Is that right?

Yes, and that row quotes the agency saying so. Faxing a document to an office that does not accept faxes loses the document with no record that you sent it, so use the channel the row names instead.
