AdventHealth Fax Numbers for Medical Records

AdventHealth publishes a fax number for every campus

AdventHealth runs hospitals, urgent care centers, imaging centers, physician practices, home health agencies, and hospices across nine states, and its Health Information Management page lists a phone number, a fax number, and an online request form for each one. There is no single national records fax, and the number that works for a patient treated in Orlando is the wrong destination for a patient treated in Denver or Shawnee Mission.

Several campuses do share a line, which is why the four numbers above cover most requests. AdventHealth Orlando and the other Central Florida hospitals use (407) 303-0633. AdventHealth Tampa and the other West Florida hospitals use (813) 615-8337. Every Centra Care urgent care visit, in every market, goes to (678) 515-5121, and every home health or hospice record goes to (678) 271-0597.

What AdventHealth patients send by fax

The paperwork that still moves by fax is the paperwork that carries a signature:

• A signed authorization for use or disclosure of protected health information, releasing your chart to yourself or to another provider
• A records request when you are moving to a clinic outside the AdventHealth system
• Disability, FMLA, or leave paperwork an AdventHealth physician has completed and signed
• Outside records going the other way, when a non-AdventHealth provider needs history added to your AdventHealth chart

AdventHealth wants a separate authorization for each physician you want records sent to, and it asks you to include that physician’s name, mailing address, phone number, and fax number on the form. A completed authorization plus an ID copy is usually two to four pages, which is inside the free monthly allowance on every FaxTerra account.

Verify the number before you send

An authorization form carries your name, date of birth, and the dates you were treated. Confirm the destination before it leaves your hands:

• Match the number to the facility that actually treated you, not to the AdventHealth brand. The table is organized by campus for a reason.
• A hospital stay and a visit to an AdventHealth physician practice produce two different records held by two different departments, each with its own fax line.
• Birth and death certificates do not come from AdventHealth at all. Those are ordered from the state Bureau of Vital Statistics.
• If you have a letter or form from AdventHealth in front of you, use the fax number printed on it. That number belongs to the department expecting your document.

How to fax AdventHealth without a fax machine
