AdventHealth routes records by facility, and lists every line

AdventHealth runs hospitals, physician practices, imaging centers, urgent care clinics, home health agencies and hospices across Colorado, Florida, Georgia, Illinois, Kansas, Kentucky, North Carolina, Texas and Wisconsin. Its Medical Records page has a tab per state and a table per facility type, with a phone, a fax and a request form for each entry. The table above is read straight from those tables, one row per fax number, with the facilities that share a line listed together.

Pick the row for the facility that treated you. A signed authorization faxed to Orlando for a stay in Shawnee Mission reaches a department that has no chart to release, and nobody forwards it. Four groups share a line and cover most requests: the Central Florida hospitals use (407) 303-0633, AdventHealth Tampa, Connerton and Heart of Florida use (813) 615-8337, every Centra Care urgent care visit goes to (678) 515-5121, and every AdventHealth home health or hospice agency goes to (678) 271-0597. Every other hospital has its own line, including the rest of West Florida (Carrollwood, Dade City, Lake Placid, North Pinellas, Ocala, Sebring, Wauchula, Wesley Chapel, Zephyrhills, Riverview) and every hospital outside Florida.

Three splits catch people. A visit to an AdventHealth Medical Group practice is a physician practice record with its own line, separate from the hospital even when the practice sits inside it. An imaging center appointment is an imaging record with its own line. And two numbers differ by one digit: Lake Placid is (866) 975-9445 while Sebring and Wauchula share (866) 375-9445, so copy the row, not a neighbour. The former AdventHealth Care Centers, now run by Solaris Healthcare, Prairie Ridge and Park Bend, keep their own rows at the end of the table for records from a stay while AdventHealth ran them.

Two entries on AdventHealth's page have no fax at all: ShorePoint Health Punta Gorda and AdventHealth Well 65+ are listed with a phone number only, so a records request for either starts with a call.

What AdventHealth says about faxing records to your doctor

AdventHealth's page states the return route: "If your physician is on staff at an AdventHealth facility, he or she may access your medical records from their office without your permission. If not, your doctor's office can be faxed records that you personally request at no charge to you." So the fax line gets your request in, and if the records are going to a physician outside AdventHealth, name that office and its fax number on the authorization and AdventHealth faxes them there without a fee. Your own copy comes back the way the form specifies, and AdventHealth asks for a separate authorization for each physician you want records sent to, with that physician's name, mailing address, phone number and fax number on the form.

On timing, the page says: "Paper copies of medical records may be released upon receipt of written authorization of patients over the age of 18 or a legal guardian. Once authorization is received, it may take up to 10 days to process your request." Keep the delivery confirmation from your fax as the receipt while that window runs.

What you are likely sending

The paperwork that still moves by fax is the paperwork that carries a signature:

  • A signed authorization for use or disclosure of protected health information, releasing your chart to yourself or to another provider
  • A records request when you are moving to a clinic outside the AdventHealth system
  • Disability, FMLA, or leave paperwork an AdventHealth physician has completed and signed
  • Outside records going the other way, when a non-AdventHealth provider needs history added to your AdventHealth chart

A completed authorization plus an ID copy is usually two to four pages, which is inside the free monthly allowance on every FaxTerra account. Birth and death certificates do not come from AdventHealth at all; those are ordered from the state vital statistics office.

How to fax AdventHealth without a fax machine

What is the AdventHealth fax number for medical records?

It depends on which AdventHealth facility treated you. The Central Florida hospitals (Orlando, Altamonte Springs, Apopka, Celebration, East Orlando, Kissimmee, Minneola, Winter Garden, Winter Park and AdventHealth for Children) share (407) 303-0633. AdventHealth Tampa, Connerton and Heart of Florida share (813) 615-8337. Centra Care urgent care records go to (678) 515-5121 and home health or hospice records to (678) 271-0597. Which number do I use if I was treated at more than one AdventHealth campus?

Send a separate request to each one. Health Information Management is organized by campus, and a request faxed to Orlando does not reach Tampa. If the visits were a hospital stay and a physician practice appointment, that is also two separate requests, because AdventHealth Medical Group records have their own line.

Can AdventHealth fax my records to my new doctor?

Yes. AdventHealth states that your doctor's office can be faxed records you personally request at no charge to you, as long as you submit the authorization form and include the physician's name, mailing address, phone number and fax number. It wants a separate form for each physician, and says processing can take up to 10 days once the authorization arrives.

Where do I send a request for an AdventHealth hospital outside Florida?

Each one has its own line in the table above: the five Colorado hospitals, Gordon, Murray and Redmond in Georgia, the four UChicago Medicine AdventHealth hospitals in Illinois, the four Kansas hospitals, Manchester in Kentucky, Hendersonville in North Carolina, Central Texas and Rollins Brook in Texas, and Durand in Wisconsin. Do I need a HIPAA plan to fax my own medical records?

No. HIPAA obligations sit with the covered entity receiving your records, not with you as the patient sending your own. A free FaxTerra account is enough.
