Tampa General will not fax the records back to you

So the request form asks you to pick a return format up front: paper by mail, a CD, encrypted email, your MyChart account, or pickup in person during Release of Information business hours. There is no charge when records go directly to your physician or reach you through MyChart. Copies mailed to you can carry a fee for labor, supplies, and postage, and Tampa General says it will tell you the total before providing them.

What to put on the request

The access form asks for what Release of Information needs to find your chart without calling you back:

  • Your full name, date of birth, the last four digits of your Social Security number, and a phone number
  • Which Tampa General or Tampa General Medical Group location treated you
  • The dates of treatment, or a date range
  • The specific records you want: an abstract report, discharge summary, history and physical, operative report, lab results, radiology or cardiology reports, progress notes, consultation reports, or an emergency room report
  • Your signature, plus your relationship to the patient if you are signing as a legal guardian or personal representative
