Which Atrium Health line you need

Atrium Health consolidated most of its records requests into one corporate Release of Information office in Charlotte. Both of its patient forms, the Authorization for Release of Health Information and the Patient Request for Health Information, list the same three ways to return a completed form: email to MedicalRecordsROI@atriumhealth.org, fax to (704) 446-6037, or mail to PO Box 32861, Charlotte, NC 28232.

That corporate line also covers the Atrium Health Wake Forest Baptist facilities in Winston-Salem, Davie, Lexington, and Wilkes. Two exceptions publish their own numbers: High Point Medical Center at (336) 878-6100 and Atrium Health Navicent in Macon, Georgia at (478) 633-7818.

Atrium Health is part of Advocate Health, which is why the Release of Information email addresses on those pages end in advocatehealth.org. The fax numbers are unchanged by that.

What Atrium Health patients send by fax

Atrium uses two different forms, and picking the wrong one is the most common reason a request comes back:

• The Patient Request for Health Information is for getting your own records, or directing them to someone you name
• The Authorization for Release of Health Information is for releasing records to an employer, an attorney, a school, or another organization
• A Health Information Amendment Form, if something in your chart is wrong
• Disability, FMLA, or leave paperwork an Atrium physician has completed and signed

Atrium asks for one patient per form and does not accept a typed signature. If the patient cannot sign, an authorized representative can, and supporting documentation may be required. A signed form plus a photo ID copy is two to four pages, which sends free.

Verify the number before you send

A completed request carries your name, date of birth, treatment dates, and the facility that treated you. Confirm where it is going first:

• Read the fax number in the submission section of the instruction sheet you downloaded from atriumhealth.org. Atrium prints it in step 9 of the release instructions and step 8 of the patient access instructions.
• If your care was at High Point or at a Macon facility, use that site's own number rather than the corporate one.
• Atrium staffs a records phone line, listed on the same page, and will confirm the destination if you ask.
• Skip fax numbers from third-party form sites. Several host an old copy of the Atrium form from before the corporate office consolidated.
