Quest Diagnostics Fax Number for Lab Records

One form, one fax number

Quest Diagnostics handles patient record requests through a single document, the Patient Request to Access or to Disclose Protected Health Information, which Quest calls the Access Form. Section F of that form gives three ways to return it: mail to Quest Diagnostics, 9601 Renner Blvd, Lenexa, Kansas 66219, ATTN Clinical Client Services; email; or fax to (855) 854-9151.

Quest states it responds within 30 days of receiving the completed form. Faxing is the fastest of the three because there is no mail transit and no encrypted-email account to set up on the recipient’s side.

What to send Quest

The Access Form asks for the details that let Quest find your results:

• Your name, including the name you used at the time of service if it differs, plus date of birth and both current and prior addresses
• Which records you want: lab test results, the order form, or something else you specify
• The ordering physician or office and the approximate dates of service
• How you want the records delivered, and to whom if you are directing them to a doctor or a third party
• Your signature, and proof of authority if you are a parent, guardian, or representative

The completed form runs two pages. If you are attaching proof of representation, expect three or four. That is inside the free monthly pages on every account.

Verify the number before you send

Your Access Form carries your name, birth date, the last four of your Social Security number, and your insurance ID. Check the destination before you send it:

• Download the Access Form directly from questdiagnostics.com and read the fax number in section F of the form you actually downloaded. It is printed on the form itself.
• If a Quest representative gives you a different number over the phone, use theirs and note who gave it to you.
• Skip fax numbers from third-party form sites. Several republish an old copy of the Quest form.
