One form, one fax number
Quest Diagnostics handles patient record requests through a single document, the Patient Request to Access or to Disclose Protected Health Information, which Quest calls the Access Form. - How you want the records delivered, and to whom if you are directing them to a doctor or a third party
- Your signature, and proof of authority if you are a parent, guardian, or representative

Verify the number before you send
Your Access Form carries your name, birth date, the last four of your Social Security number, and your insurance ID. Check the destination before you send it:
- Download the Access Form directly from questdiagnostics.com and read the fax number in section F of the form you actually downloaded. It is printed on the form itself.
- If a Quest representative gives you a different number over the phone, use theirs and note who gave it to you.
- Skip fax numbers from third-party form sites. Several republish an old copy of the Quest form.
