Baptist Health Fax Numbers for Medical Records

Baptist Health is several separate health systems

"Baptist Health" is not one organization. Six unrelated health systems use the name, each with its own website, its own Release of Information department, and its own fax lines:

• Baptist Health in Kentucky and southern Indiana, headquartered in Louisville
• Baptist Health in Jacksonville, Florida
• Baptist Health South Florida, in the Miami area and up the coast to Boca Raton
• Baptist Health in Arkansas, headquartered in Little Rock
• Baptist Health in Alabama, in the Birmingham area
• Baptist Health System in San Antonio, Texas, with a seventh hospital in New Braunfels

None of them can pull a record held by another, and a request faxed to Louisville for care received in Miami is not forwarded. It arrives at a records department that has no chart matching your name.

Start from the hospital, not from the brand. If you have a discharge summary, a bill, or an appointment reminder, the city on it tells you which system to write to, and within the Texas and Florida systems, which hospital.

How each system routes records

The systems differ in more than the number:

• Kentucky and southern Indiana takes the completed form by mail or fax at one Release of Information line for every hospital, and states it cannot accept electronic signatures on the form. Sign it by hand.
• Jacksonville takes the completed form by mail, fax or email at its Release of Information Office, and routes outpatient behavioral health records, radiology records and mammography records through separate lines per campus, all listed above. It also publishes a fax for providers requesting hospital records for continued care; that route is for the provider, not for you.
• South Florida splits by facility. Every Miami-area facility uses one Health Information Management line, and Bethesda Hospital East, Bethesda Hospital West and Boca Raton Regional Hospital share another. Attach a copy of a photo ID; the page says processing can take up to 15 business days.
• Arkansas prints one Health Information Management fax beside every one of its hospitals, with a different phone number for each, and states that it accepts electronic signatures on release forms.
• Alabama asks you to download the release form for your hospital and mail or fax it to that hospital, but the form links on its records page do not currently resolve. The Brookwood form is the one Alabama form still served, so Brookwood is the only Alabama line above.
• San Antonio publishes a Release of Information fax for each of its seven hospitals and says you can submit the authorization by fax or mail to any of them. Send it to the hospital that treated you, and expect a fee for a copy after discharge.

What you are likely sending

• The signed authorization or release form from the system that treated you, releasing records to yourself, a new provider, an insurer or an attorney
• An imaging or mammography request in Jacksonville, which goes to the campus imaging line rather than the Release of Information Office
• A records request ahead of a second opinion or a move, where the receiving clinic has given you a deadline
• Outside records going the other way, so a Baptist Health clinic has your prior history

A signed authorization carries your name, date of birth, and the treatment dates you are asking about. Download the form from the site of the system you are writing to, since the website you took the form from is the system you are writing to, and several Baptist Health systems print the fax number on the form rather than on the page. If you were treated by two different Baptist Health systems, that is two separate requests to two separate organizations. A signed form plus an ID copy is two to four pages, which sends free.
