Baptist Health is several separate health systems

“Baptist Health” is not one organization. At least five unrelated health systems use the name, each with its own website, its own Release of Information department, and its own fax number:

  • Baptist Health in Kentucky and southern Indiana, headquartered in Louisville
  • Baptist Health in Jacksonville, Florida
  • Baptist Health South Florida, in the Miami area
  • Baptist Health in Arkansas, headquartered in Little Rock
  • Baptist Health in Alabama, in the Birmingham area

There is also a Baptist Health System in San Antonio, Texas. None of them can pull a record held by another, and a request faxed to Louisville for care received in Miami is not forwarded. It arrives at a records department that has no chart matching your name.

Find the one that treated you

Each system publishes its own route, and they differ in more than the number:

  • Kentucky and southern Indiana takes the completed form by mail or fax, and states it cannot accept electronic signatures on the form. Sign it by hand.
  • Jacksonville offers an online authorization form as well as mail, fax, and email, and routes behavioral health outpatient records through a separate department.
  • South Florida splits by facility. The Miami-area facilities use one line, and the Boynton Beach and Boca Raton hospitals use another, published on the same page.
  • Arkansas shares one Health Information Management fax across all its hospitals, with a different phone number for each.
  • Alabama publishes a separate release form per hospital, each carrying its own fax number in the submission block.

Verify the number before you send

A signed authorization carries your name, date of birth, and the treatment dates you are asking about. Confirm the destination first:

  • Confirm the system before the number. The website you downloaded the form from is the system you are writing to.
  • Download the release form from that system’s own site and use the submission details printed on the form, since several Baptist Health systems publish the number on the form rather than on the page.
  • If you were treated by two different Baptist Health systems, that is two separate requests to two separate organizations.
  • Skip third-party directories entirely here. Numbers for one Baptist Health are routinely listed under another, which is the single most common way a request goes to the wrong state.
