BayCare does not take records requests by fax
Neither half of BayCare accepts a records request by fax. The BayCare Medical Group authorization, which covers visits to a BayCare physician office, tells you to return it by emailing it to BMGMedicalRecordRequest@baycare.org or returning it to your BMG office. The hospital authorization is returned by mail to the hospital that treated you, and BayCare lists a Release of Information phone number and address for each hospital without publishing a fax number for any of them.
Older copies of the BayCare Medical Group form did carry a fax number in the return block. BayCare removed it, and the revision the site links today lists only email and the practice itself. If you find a BayCare records fax number on a directory site, it is coming from a retired form and not from BayCare. A signed authorization sent to a retired line goes somewhere neither you nor BayCare controls, and the request itself never arrives.
How to request your BayCare records
Which form you need depends on who treated you, not on which BayCare building you visited.
- A BayCare Medical Group physician office. Use the BayCare Medical Group Release of Information Authorization. Email the completed form to BMGMedicalRecordRequest@baycare.org, or hand it to the practice.
- A BayCare hospital. Use the hospital authorization and mail it to the hospital where you received services. Each hospital has its own Release of Information department with its own phone number and address on the BayCare medical records page. Call that department if you need to arrange anything other than mail.
BayCare says it typically takes under 5 days to fulfil a request, with more time needed for some. Delivery to your BayCare portal account or to your email is free; mailed paper copies carry per-page copying charges.
BayCare will still fax records to another provider
The fax direction that works with BayCare is outbound. BayCare states it will fax or mail your records at no charge to your doctor's office or treatment center, so if the point of your request is getting records to another provider, you do not need to move the paper yourself.
Name the receiving office and its fax number in the disclosure section of the authorization. Both BayCare forms have a field for it. That is the fastest route when a specialist needs your history before an appointment, and it avoids you handling the records at all.
