BayCare handles doctor's office records and hospital records through different departments, and only one of them publishes a fax number. Here is where your request should go, and how to send it free from your phone.

Office records fax, hospital records do not
BayCare splits records requests in two. A visit to a BayCare Medical Group physician office uses the BayCare Medical Group Release of Information Authorization, and that form can be faxed to (727) 333-6335, emailed to BMGMedicalRecordRequest@baycare.org, or handed back to the practice.
A hospital stay is different. BayCare's hospital release form is returned by mail to the hospital where you received services, and BayCare lists a phone number and mailing address for each hospital's Release of Information department without publishing a fax number for any of them. If you need to send a hospital request by fax, call that hospital's Release of Information department and ask for a number.
BayCare states it generally takes 4 to 5 business days to fulfil a records request, and that delivery to a BayCare portal account or to your email is faster than mail.
