How to Find the NYU Langone Fax Number for Medical Records

NYU Langone does not publish a records fax number

NYU Langone's own records page tells patients to complete the Authorization for Use and Disclosure of Protected Health Information and submit it by traditional mail, fax or email. It then lists what it does publish: a Health Information Management mailing address at 1 Park Avenue, 3rd Floor, New York, NY 10016, and a Release of Information email address for each hospital.

The authorization form itself carries an instruction chart of sites, addresses and telephone numbers, again with no fax number. The only fax field on the form is the blank where you write the number you want your records sent to.

That is worth saying plainly, because pages elsewhere assert an NYU Langone records fax number. A signed authorization sent to a number NYU Langone does not staff is not being worked, and nobody will tell you. Get the number from NYU Langone before you send.

How to get the right number for your site of care

NYU Langone splits Health Information Management by hospital, so the number has to come from the team that holds your chart:

• Email the Release of Information address for your hospital, which NYU Langone lists on its records page, and ask for the fax number for submitting an authorization. Tisch, Kimmel Pavilion, Hassenfeld Children's Hospital and Perlmutter Cancer Center share one address; Brooklyn, Long Island, Suffolk and the Orthopedic Hospital each have their own.
• Call the Health Information Management phone number printed on the authorization form for your site. NYU Langone prints a telephone number for each of those sites in the instruction chart at the front of the form, including separate lines for the main Manhattan hospitals, the Orthopedic Hospital, Long Island, Perlmutter Cancer Center and Brooklyn.
• If you were seen by a Faculty Group Practice or a Family Health Center, NYU Langone directs the request to the individual office. Ask that office for its own fax number.
• Radiology films and images are handled by the imaging location, not by Health Information Management.

Write the number down with the name of the person who gave it to you and the date. That is the record you will want if the request stalls.

What you are likely sending

• The signed Authorization for Use and Disclosure of Protected Health Information, releasing your chart to yourself or to a new provider
• Outside records going the other way, when an NYU Langone physician needs history from a clinic you saw elsewhere
• Disability, FMLA or school paperwork an NYU Langone physician has completed
• Prior authorization or appeal paperwork headed to your insurer rather than to NYU Langone

NYU Langone's form asks you to initial separately for alcohol or drug treatment information, mental health treatment information, genetic information and confidential HIV-related information. If any of those apply and the initials are missing, the request comes back. An incomplete form is rejected outright: NYU Langone states that incomplete forms will not be accepted.
