Which Capital One fax line you need
All three published numbers belong to Capital One Bank, the checking and savings side of the business.
Banking forms and general correspondence go to (877) 650-3528, the number on the Capital One banking forms page. Beneficiary designations and the bank power of attorney forms listed there are submitted this way, with the cover sheet addressed Attn: CORR.
A name change on a checking or savings account goes to (888) 464-3220 with the document that proves it and a government ID.
