Pharmacy records and clinic records are separate

A CVS store can hold two different kinds of record about you. Your prescription history belongs to CVS Pharmacy. A visit to the MinuteClinic inside the same store belongs to MinuteClinic, a separate provider entity with its own privacy office. They use different authorization forms and different fax numbers.

CVS Pharmacy’s Authorization for Disclosure of Protected Health Information tells you to mail the completed form to One CVS Drive, MC-B120, Woonsocket, RI 02895 or fax it to (401) 652-1593. Sending a prescription-history request to the clinic line does not fail loudly; it just sits.
