CVS Caremark Fax Number

Caremark’s prescription fax is prescriber-only

CVS Caremark’s FastStart form, the one used to send a maintenance prescription to the Caremark Mail Service Pharmacy, carries the rule on the page: the fax “will only be accepted when sent from a prescriber’s secure fax line.” A prescription faxed by a member is not filled, and no one tells you.

Member requests for prescription records go a different route again. Caremark’s HIPAA Notice of Privacy Practices asks for a written request to the CVS Health Privacy Office, One CVS Dr., Woonsocket RI 02895, and publishes no fax for it. So this page asserts no Caremark fax number for members, because Caremark publishes none.

What to do instead

For getting a prescription filled by mail:

• Ask your prescriber to send it electronically to CVS Caremark Mail Service Pharmacy, which is the route Caremark’s own form recommends first
• Order a refill of an existing prescription in your Caremark account or app
• Call the number on the back of your prescription ID card

For getting your prescription records:

• Your Caremark account shows claims history and a payment summary you can print
• A written request to the CVS Health Privacy Office covers periods the account does not
• If the records you want are retail prescriptions filled at a CVS store, that is CVS Pharmacy, a different entity with its own form and its own fax number

What you can fax about a Caremark benefit

Coverage paperwork, once your plan has given you the destination:

• A prior authorization or exception request, when a drug is not covered as written
• An appeal of a denied drug claim, using the number on the denial letter or in your plan documents
• Documents Caremark or your plan has specifically asked you to send

Caremark administers benefits for many different health plans, and the fax number for a prior authorization is set by the plan, not by Caremark centrally. The letter you received names it. That is why a general Caremark prior-authorization number found by searching is usually the wrong one.
