Geisinger Fax Number for Medical Records

One department, two ways in

Geisinger centralizes records requests rather than splitting them by hospital. Its instruction on the medical records page is to fax or mail the completed Authorization to Release Medical Information form to Health Information Management, Release of Medical Information, 100 N. Academy Ave., Danville, PA 17822-1311, with the fax number (570) 214-9523.

Geisinger's separate instruction sheet for the same form lists a second line into the same Centralized Release of Medical Information department, (570) 808-6063, and says to fax the completed form to one of the two. Either reaches the same team. Mail can also go to Geisinger Wyoming Valley Medical Center, Attn: Release of Medical Information, 1000 E. Mountain Drive, Wilkes-Barre, PA 18711-3845.

Primary care physician records are the exception. Geisinger says those may be requested directly from your care site rather than through the central department.

What patients fax Geisinger

• The signed Authorization to Release Medical Information, when the records go to someone other than you or your legal representative
• The Patient Right of Access form, when you want a copy of your own record
• The Authorization to Request Medical Information from Another Facility, when you want Geisinger to pull records in from a clinic you saw elsewhere
• Disability, FMLA or school paperwork a Geisinger physician has completed

Geisinger's form asks you to select the Geisinger entities you are requesting from, so a request that spans a hospital and a clinic can go on one form. The authorized recipient section requires a full name, complete address, phone number, and a fax number and email address where applicable. Verify the number before you send

The authorization names you, your date of birth and your medical record number, and it names who receives your chart. Confirm the destination first:

• Read the fax number off the Geisinger medical records page or off the instruction sheet that comes with the form you downloaded, rather than from a search result.
• Both published numbers reach the same department, so either is fine. Use the one on the newer of the two documents if they ever disagree.
• Call the Geisinger System Release of Medical Information call center listed on the instruction sheet and select option 6 to reach a release of medical information specialist if anything is ambiguous.
• If the records you want are from a primary care office, ask that office first. Geisinger routes those requests to the care site.
