IU Health has no single records fax for the system. Each Health Information Management office covers a group of hospitals, and IU Health's instruction for the completed authorization is to fax, email or mail it to the office for the facility that treated you.
Most requests land in one of four places. Care at Methodist Hospital, University Hospital, Riley Hospital for Children, North, West, Fishers, an Ambulatory Surgery Center or an IU Health Medical Group office goes to the Metro Indiana line at (317) 968-1177. Bloomington, Bedford, Morgan and Paoli share (317) 968-1413. Ball Memorial, Blackford and Jay share (765) 448-7620. Frankfort, Tipton and White Memorial share (317) 968-1321.
IU Health Arnett hospitals and clinics in Lafayette are handled by their own office at 2600 Greenbush, and are the one group where the fax numbers on the IU Health page carry an out-of-state area code that does not match the Indiana phone number beside them. Call the Arnett Release of Information office, using the phone number IU Health lists beside it, and have them read the fax number back before you send anything there.
IU Health's goal is to process a request within five business days, though state and federal law allow up to 30.
