Two organizations, two forms, two fax numbers
HealthPartners provides care and sells insurance, and the records question splits along that line.
If you want your chart, that is the care system: HealthPartners and Park Nicollet clinics, Regions Hospital, Methodist Hospital, TRIA, and the Amery, Hudson, Hutchinson, Lakeview, Olivia, Stillwater, and Westfields hospitals and clinics. The form is the Patient Authorization for Release of Protected Health Information, and the Release of Information department it names sits at 3800 Park Nicollet Blvd. in St. Louis Park, reachable at (952) 883-9714.
If you want your plan record, that is the insurer: membership and member ID, claims and payments, medical management and authorizations, complaint and appeal outcomes, and FSA or HRA balances. The form is the Member Authorization for Release of Protected Information, and HealthPartners asks for it by mail to Mail Stop 21104G in Minneapolis or by fax at (952) 883-5666.

What to put on the care-system form
The Patient Authorization form is built around four questions, and answering them precisely is what keeps a request from bouncing:
- Who has the information. You can name the whole HealthPartners organization or a single facility within it. No authorization is needed to move records between two HealthPartners facilities.
- Where it goes. An individual, a business, or another health care organization, with as much detail as you have.
- What you need. A summary of care, individual documents such as a discharge summary or an operative report, or records that need special permission, which are itemized separately.
- How it is delivered. Email, the online patient portal, mail, or fax. HealthPartners notes that a mailed request over 75 pages arrives on an encrypted CD, and that portal delivery is unavailable in some proxy situations, in which case you should ask for mail, fax, or secure email.
Paper charts stored offsite are not part of the standard record set for an “any and all” request, so ask for them by name if you need them. An authorization is valid for one year unless you set a different date.
Verify the number before you send
Both forms carry your name, date of birth, and enough detail to identify your care. Confirm the destination first:
- Download the form from healthpartners.com and read the contact section on the back page of the form you actually downloaded. HealthPartners revises these forms, and the current care-system form is dated 12/2025.
- Check which side you need. A chart request faxed to the health plan will not be worked by the people who hold the chart.
- Call the Release of Information department on the phone number printed beside the fax number on that form, and ask them to confirm the destination for your request.
- For a billing or radiology question, use the per-hospital numbers printed on the same form rather than the general Release of Information line.
