SSM Health routes records by state, then by hospital

SSM Health operates in Missouri, Illinois, Oklahoma, and Wisconsin, and its records page is organized the same way. Set your location and the page loads the forms and the contacts for that region, then lists a Health Information Management block for each hospital: a mailing address, a fax number, an email address, and a phone number.

SSM Health uses MyChart, and it will show test results, visit notes, and much of the recent record without a form. If you need last month's lab work or a visit summary, that is the fastest route and it costs nothing.

The form matters when MyChart does not go far enough: older records, a complete chart, radiology images, records for someone you are the legal representative for, or records that have to go to a third party such as an attorney or a new clinic.
