SSM Health routes records by state, then by hospital

SSM Health operates in Missouri, Illinois, Oklahoma and Wisconsin, and its records page is organized the same way. Set your location and the page shows a block for each hospital in that region: a mailing address, a fax number, an email address and a phone number. The table above is read from those blocks, one row per fax number, grouped by state.

Pick the row for the hospital that treated you. In St. Louis that means one of seven separate offices: Cardinal Glennon, DePaul at (314) 344-6598, Saint Louis University Hospital, St. Clare in Fenton, St. Joseph in Lake Saint Louis, St. Joseph in St. Charles, and St. Mary's in Richmond Heights. Two SSM Health regions run one office for several facilities: the Oklahoma City line at (405) 231-8897 covers St. Anthony Hospital, Bone & Joint Hospital at St. Anthony, the St. Anthony Healthplex locations and SSM Health Medical Group in Oklahoma, and the Wisconsin line at (610) 879-3704 covers Dean Medical Group, Monroe Hospital, St. Clare Baraboo, St. Mary's Janesville and St. Mary's Madison, with "Attn: Release" on the cover sheet.

Two offices publish no fax at all. SSM Health's hospital records office in Fond du Lac, Wisconsin, takes requests by mail and email only, and SSM Health Medical Group and SLUCare Physician Group offices in St. Louis and the Metro East take requests through an online form or by phone to SSM Health's records vendor. A hospital stay and a physician group visit in St. Louis are therefore two different requests through two different routes.

What SSM Health says about the request

SSM Health publishes no restriction on faxing. Its page heads the contact section "Request Medical Records via Mail, Fax, or Email" and tells you to navigate to your facility for the records representative's contact details, which is what the table above does for you. The blocks ask for specific cover-sheet wording, and it matters: Illinois hospitals want "ATTN: HIM Department", Jefferson City wants "Attn: Release of Information/HIM", the St. Louis hospitals want "Attn: Health Information Management", Oklahoma wants "ATTN: Medical Records", and Wisconsin wants "Attn: Release". Each row's note carries the wording for that office.

Try MyChart first if the record is recent

SSM Health uses MyChart, and it will show test results, visit notes, and much of the recent record without a form. If you need last month's lab work or a visit summary, that is the fastest route and it costs nothing.

The form matters when MyChart does not go far enough: older records, a complete chart, radiology images, records for someone you are the legal representative for, or records that have to go to a third party such as an attorney or a new clinic. SSM Health also offers an electronic request per region, which validates your identity and captures the authorization online.

So the honest order of operations is portal, then online request, then the form by fax. Faxing wins when you already have a signed form in hand and a deadline in front of you.

The documents that travel this way carry a signature:

• The Authorization for Release of Protected Patient Health Information, in the version for your state
• A records request when you are changing providers and the next clinic needs your history
• Guardianship, power of attorney, or health care proxy paperwork attached to a request made on someone else's behalf
• Disability, FMLA, or leave forms an SSM Health physician has completed and signed
• Behavioral health or home health authorizations, which SSM Health publishes on separate forms
