HonorHealth Fax Numbers for Medical Records
HonorHealth centralizes records requests at one Release of Information office for the whole network, with a second line for Cancer Care records. Both numbers below are read from HonorHealth's own records page and the January 2025 form it links. Which line is yours is set by the kind of care, and patients from four East Valley campuses mail the form instead.

One network office, one cancer care line
HonorHealth centralizes records requests at Network Support Services in Phoenix rather than at each hospital. A medical group, urgent care or hospital record goes to the general Release of Information line at (480) 882-5841, using the Medical Records Request form HonorHealth links from its records page. HonorHealth Cancer Care keeps its own form and its own line at (480) 882-5875. The numbers above are read from HonorHealth's Request Medical Records page, and the general line is printed again in the submission block of the January 2025 form.
Two other requests go to the general line as well. An amendment request, when you believe something in your chart is wrong, uses the Amendment Request Form. An opt-out of health information sharing uses its own form. Both are addressed to Network Support Services at 2500 W. Utopia in Phoenix.
HonorHealth also offers an online route through a records vendor, where you upload the signed authorization and the records are ready in five to seven business days. An earlier version of the release form listed a separate Health Information Management office and fax at each campus; HonorHealth has replaced it with the single centralized form, so a campus number from an old copy of the form or a third-party site is not one HonorHealth publishes today.
Four East Valley campuses are mail-only
The January 2025 form carries one routing rule the records page does not. For patients seen at Mesa Emergency Center, Mountain Vista, Florence or Tempe Medical Centers, it says: "please mail a completed authorization to 1301 S Crismon Rd. Mesa, Az. 85209 Attn: Medical Records". So if your care was at one of those four, HonorHealth asks for the completed authorization by mail to its Mesa records office rather than by fax to Phoenix. Everyone else in the network uses the fax lines above.
HonorHealth also states that, on request, it sends pertinent records to your provider for continued care at no charge, so if the records are going to another doctor, name that office and its fax number in the delivery section of the form.
What patients fax HonorHealth
- The signed Medical Records Request form, releasing your chart to yourself, a new provider, an attorney or an insurer
- A request to send imaging reports, lab results, operative reports or a discharge summary to a specialist ahead of an appointment
- The Amendment Request Form when a diagnosis, medication or demographic detail in your record is incorrect
- Disability or leave paperwork completed by an HonorHealth physician
HonorHealth's authorization expires on completion or 60 days from the date you sign it, whichever comes first, so a form you signed months ago and never sent has to be redone. A signed release plus an ID copy is usually two to three pages, which sends free.
Verify the number before you send
Your release form carries your name, date of birth, address and dates of service. Confirm the destination first:
- Download the form from honorhealth.com and read the fax number in the submission block of the form you actually downloaded, then check it against the records-request page.
- Cancer Care records use the Cancer Care form and the Cancer Care line. Sending a Cancer Care request to the general number adds a hand-off you do not need.
- If you were seen at Mesa Emergency Center, Mountain Vista, Florence or Tempe, the form says to mail it, not fax it.
- Call the Health Information Management phone number on the records-request page and ask them to read the fax number back if anything is ambiguous.
- Skip fax numbers from third-party form sites. Several republish the older four-campus revision of the HonorHealth form.

Campus-specific fax numbers come from an older revision of the form that HonorHealth no longer links.
How long is an HonorHealth authorization good for?
HonorHealth states the authorization expires upon its completion or 60 days from the date of signature, whichever comes first. Sign and send in the same sitting rather than holding the form.
Can I fax records to HonorHealth instead of requesting them?
Yes. Outside records that a new HonorHealth physician needs go to the same Release of Information office. Put the patient name, date of birth and the receiving physician on the cover sheet so it can be matched to the right chart.
Do I need a HIPAA plan to fax my own medical records?
No. HIPAA obligations sit with the covered entity receiving your records, not with you as the patient sending your own. A free FaxTerra account is enough.
