Labcorp does not publish a records-request fax
Labcorp’s HIPAA Notice of Privacy Practices, effective February 10, 2026, lists exactly four ways to get a copy of your protected health information: ask for a courtesy copy at a Labcorp patient service center, open a patient portal account to receive your reports electronically, complete Labcorp’s HIPAA Patient Request Form, or contact Labcorp’s Privacy Officer. Fax is not one of them.
That is worth saying plainly, because plenty of pages elsewhere assert a Labcorp records fax number. Faxing a signed authorization to a number Labcorp does not staff means your request is not being worked and you will not be told. Use one of the four routes above for records.
What Labcorp does take by fax
Billing. Labcorp’s patient help pages ask you to fax a copy of your Explanation of Benefits when a Labcorp bill does not match what your insurer processed. The number is (866) 227-2939, and the mail alternative is P.O. Box 2240, Burlington, NC 27216-2240.
Include your Labcorp invoice number on the cover sheet. Without it the billing team has to match your document to an account by hand, which is the most common cause of a slow answer. An EOB plus a cover sheet is usually two or three pages, which sends free.
How to get your Labcorp results
For results rather than billing:
- The Labcorp Patient portal shows your reports as they are released, with no form to complete
- A courtesy copy at any Labcorp patient service center is free and immediate if you are nearby

- Your ordering physician already has the report and can usually hand you a copy the same day
If your goal is to move results to a new provider, ask the new provider’s office to request them. Provider-to-provider requests use a different route and are usually faster than a patient request.
