Three records offices, three numbers

M Health Fairview asks you to pick where you were treated before it gives you a number, and it publishes three separate records offices for that reason.

Most patients want the first one. M Health Fairview Hospitals and Clinics covers the Twin Cities network, takes the Authorization for Release of Protected Health Information at (952) 915-8824, and also accepts the request by email at releaseofinformation@fairview.org. Fairview Range in Hibbing runs its own Health Information Management office at (218) 362-6678. Grand Itasca Clinic & Hospital in Grand Rapids uses (218) 999-1513.

One thing M Health Fairview states next to every one of those numbers: staff at these numbers will not share your health information verbally. The line is for submitting the form and asking process questions, not for getting results read out.

What M Health Fairview patients send by fax

The requests that need a signature and a destination:

• A signed Authorization for Release of Protected Health Information, releasing your chart to yourself or to a new provider
• A records request when you are leaving the system and your next clinic needs your history
• A Request for Amendment to Health Information, when something in the chart is wrong. That one goes to the address printed on the form rather than to release of information.
• Disability, FMLA, or leave paperwork an M Health Fairview physician has completed and signed
