One service, five offices
Mayo Clinic's records page shows one contact set for Release of Information: the Rochester Health Information Management Services office, with office hours listed per state. That is the number most people find, and it reaches Release of Information for any campus.
The same page links the Authorization to Release Protected Health Information to a Third Party, and that form tells a fuller story. Its second page is a contact table headed HIMS Release of Information, with five offices: Arizona in Scottsdale, Florida in Jacksonville, Rochester, Mayo Clinic Health System Minnesota in Mankato, and Mayo Clinic Health System Wisconsin in Eau Claire. Each has its own fax. Mayo Clinic Health System's own health record forms page points at the same form and says the submission instructions, including fax numbers, are on it.
So pick the row for the campus or health-system site that treated you. If you are not sure which office holds the chart or your care spanned campuses, the Rochester line is the one Mayo publishes as the general contact. The four campus lines come from the current form rather than a web page. If Mayo has sent you a letter or a form with a different number on it, use that one.
A request to correct something you believe is wrong in your record is an amendment, a separate process described on the same records page rather than a Release of Information request. Requests to obtain records are the fax route; requests to change them follow the page's own instructions.
What Mayo says about sending records in
The Release of Information offices release records; they do not collect them. Mayo states on its records page: "If you have non-Mayo Clinic medical records to send for an upcoming appointment, do not send them to the Mayo Clinic Release of Information team." Outside records for an upcoming appointment go to your care team, by attaching them to a message in the patient portal or as the care team instructs. The form itself carries the same reminder: records being sent to Mayo Clinic go to the department and fax number you name in section 5 on page 1, not to the Release of Information office.
