Mayo Clinic's records page shows a single contact set for Health Information Management Services Release of Information, the Rochester office, with office hours listed per state. That is the number most people find, and it reaches Release of Information for any campus.
The authorization form the same page links, Authorization to Release Protected Health Information to a Third Party, tells a fuller story. Its second page is a contact table headed HIMS Release of Information, with five offices: Arizona in Scottsdale, Florida in Jacksonville, Rochester, Mayo Clinic Health System Minnesota in Mankato, and Mayo Clinic Health System Wisconsin in Eau Claire. Each has its own fax. Mayo Clinic Health System's own health record forms page points at the same form and says the submission instructions, including fax numbers, are on it.
So pick the row for the campus or health-system site that treated you. Mayo states on its records page: "If you have non-Mayo Clinic medical records to send for an upcoming appointment, do not send them to the Mayo Clinic Release of Information team."
