Imaging publishes a fax number. General records do not

Memorial Hermann's Release of Information department publishes hours, a phone number, and two addresses, and no submission fax. Requests go to Memorial Hermann Release of Information, 7737 Southwest Freeway, C94, Houston, Texas 77074, or in person at 930 Frostwood, Suite 1.103. The department answers on the phone number published on that page, Monday through Friday, 8:00 a.m. to 4:00 p.m.

Imaging is different. For records that include test images on film or CD, Memorial Hermann asks you to complete the imaging form and fax or email it, at (713) 778-2577 or to its outpatient imaging customer service address. The instruction asks that the cover be marked “Attention: Request for Protected Health Information.”

Memorial Hermann responds to a records request within 15 days of receipt, and may charge a cost-based fee covering labor, supplies, and postage.

Some Memorial Hermann locations hold their own records

Worth checking before you send anything. Memorial Hermann states that it has joint venture partnerships with other health care providers for which it is not the custodian of the records, and names its surgery centers and Memorial Hermann-GoHealth Urgent Care locations. A request for records from one of those goes to that facility, not to Memorial Hermann's Release of Information department.

The practical test: if the bill came from the facility rather than from Memorial Hermann Health System, the facility probably holds the chart.

What Memorial Hermann patients send
