More than one health system uses the name, and they are separate companies with separate records departments.

Mercy publishes a fax number for every records office

Mercy's own instruction is per-facility: complete the form, then submit it to your Mercy physician's office or your local Mercy Health Information Management office. Its medical-records page then prints an address, a phone number, and usually a fax number for roughly two dozen offices, from Ada and Ardmore in Oklahoma to Berryville and Rogers in Arkansas, Pittsburg in Kansas, and a dozen Missouri towns.

Several Mercy offices in Arkansas route their records work through a records vendor, and the address on those entries is a vendor email rather than a mercy.net one. That is Mercy's own published arrangement, and the fax numbers on those entries are the ones Mercy prints.

What Mercy patients send

Mercy publishes two main forms and several smaller ones:

  • Patient Request: Access to Protected Health Information, when you want your own records
  • Authorization for Use and Disclosure of Protected Health Information, when a third party is receiving them
  • Request for an accounting of disclosures, request for an amendment, request for confidential communication, and request to restrict release to an insurance carrier, each on its own form
  • Disability, FMLA, or leave paperwork a Mercy physician has completed

Verify the number before you send

A records request carries your name, date of birth, and dates of service. Confirm the destination first:

  • Use the fax number printed on the Mercy letter or form in front of you.
  • If you have no form, open Mercy's medical-records page and read the Health Information Management block for the hospital or clinic that treated you. The blocks are listed by town.
  • Call the phone number in that same block and ask them to read the fax number back to you.
  • Confirm you are looking at mercy.net and not another Mercy-named system. A request sent to the wrong company will not reach anyone who has your chart.
