More than one health system uses the name, and they are separate companies with separate records departments.

• Mercy, at mercy.net, headquartered in Chesterfield, Missouri, with hospitals across Missouri, Arkansas, Oklahoma and Kansas. This page covers that Mercy.
• Mercy Health, at mercy.com, a different organization with its own patient-resources site.
• Mercy Medical Center in Springfield, Massachusetts, listed among Trinity Health Of New England's hospitals, and Mercy Fitzgerald and Mercy Philadelphia, listed under Trinity Health Mid-Atlantic. Records for those go through Trinity Health, covered on the Trinity Health page.

The domain on your bill or your patient portal is the quickest way to tell. If it is mercy.net, the numbers above apply.

Mercy routes records by hospital

Mercy's own instruction is per-facility: complete the form, then send it by "submitting to your Mercy physician's office or local Mercy Health Information Management office". Its Requesting Mercy Medical Records page prints a Health Information Management block for each hospital, listed by town, with an address, a phone number and usually a fax. The numbers above are read from those blocks, and the table carries every fax Mercy prints.

Pick the row for the hospital that treated you. A signed authorization sent to a records office that has no chart for you sits with a department that cannot release anything, and nobody forwards it. Two splits catch people in St. Louis: Mercy Hospital St. Louis on New Ballas Road and Mercy Hospital South on Kennerly Road are different hospitals with different records offices and different fax lines, so check the address on your paperwork. Several lines are shared on purpose. Mercy's three Arkansas hospitals route records through one records vendor with one toll-free fax, and the Oklahoma City office handles Kingfisher, Logan County and Watonga.

Two Mercy hospitals, Healdton and Tishomingo in Oklahoma, publish a phone number for records but no fax; call that office to ask how to submit. Records from a Mercy Clinic physician office have no separate fax block on the page. Mercy's instruction is to hand the form to your Mercy physician's office, or to send it to the Health Information Management office of the hospital that clinic belongs to.

What Mercy patients send

Mercy publishes two main forms and several smaller ones:

• Patient Request: Access to Protected Health Information, when you want your own records
• Authorization for Use and Disclosure of Protected Health Information, when a third party is receiving them
• Request for an accounting of disclosures, request for an amendment, request for confidential communication, and request to restrict release to an insurance carrier, each on its own form
• Disability, FMLA, or leave paperwork a Mercy physician has completed

Verify the number before you send

A records request carries your name, date of birth, and dates of service. Confirm the destination first:

• Use the fax number printed on the Mercy letter or form in front of you, if it has one.
• Match the hospital name and address on your paperwork to the row above. The St. Louis and South campuses are the ones most often mixed up.
• Call the phone number in that hospital's block on Mercy's records page and ask them to read the fax number back to you.
• Confirm you are looking at mercy.net and not another Mercy-named system. A request sent to the wrong company will not reach anyone who has your chart.
• Do not take the number from a third-party directory or a search snippet. Mercy publishes these itself, and the published version is the one to trust.
