Novant Health consolidated records requests into a single Release of Information office in Charlotte. Complete the Authorization to Disclose Protected Health or Billing Information form, then return it by fax to (704) 316-9556, by email to ROIenterprise@novanthealth.org, or by mail to P.O. Box 7688, Charlotte, NC 28241. Novant asks for a separate form per patient.

South Carolina is the exception. Novant directs all South Carolina medical and billing record requests, covering Coastal Carolina Medical Center, East Cooper Medical Center, Hilton Head Hospital, and their affiliated physician clinics, to (843) 410-2724 instead.

Third-party requests use the same two numbers. An attorney, insurer, or employer must submit a HIPAA-compliant patient authorization, and because the request comes from a representative rather than the patient, Novant applies a shorter window before a new authorization is required.

What Novant Health patients send by fax

The paperwork that needs a signature and a destination:

  • A signed Authorization to Disclose, releasing your chart to yourself, a new provider, an employer, or an attorney
  • Billing records, which the same form covers when you tick the billing option
  • A records request when you are moving out of the Novant network
  • Disability, FMLA, or leave paperwork a Novant physician has completed and signed
  • Outside records going the other way, so a Novant clinic has your prior history

Novant publishes the form in English and Spanish along with a separate instruction sheet. A signed form plus a photo ID copy is two to three pages, inside the free monthly allowance on every FaxTerra account.

  • If your care was in South Carolina, use the South Carolina line. A request sent to the Charlotte office has to be rerouted.
  • If a Novant department wrote to you, use the number in that letter.
  • Skip fax numbers from third-party form sites, several of which still list pre-consolidation hospital numbers.
