Mass General and Brigham share one records line

The merger that produced Mass General Brigham consolidated records requests along with the brand. The Massachusetts General Hospital release form, the Brigham release form and the community hospital versions are all the same document, MGB00087, and all of them print the same return block: Release of Information, 121 Inner Belt Road, Room 240, Somerville, MA 02143-4453, fax (617) 726-3661.

So a records request for Mass General, Brigham and Women's, Salem Hospital or a Mass General Brigham community hospital goes to the same place. Download the form for the hospital that treated you, because the record-type checkboxes differ slightly, then send it to the shared line.

Mass General Brigham asks patients not to email the form, on the grounds that the security of the personal information in it cannot be guaranteed that way. Fax and mail are the two routes it offers, alongside requesting records inside Patient Gateway.

Imaging is the exception

Copies of radiology images and films are handled by the imaging department at the hospital that performed the study, not by Release of Information.   • Reports and images go to different places. Use the Release of Information line for records and the imaging contact for films on disc.
  • Call the Release of Information questions line printed on the form and ask them to confirm the fax number.
  • Do not email the completed form. Mass General Brigham states directly that the security of personal information submitted by email cannot be guaranteed.
  • Skip fax numbers from third-party form sites. Several still publish separate Mass General and Brigham numbers from before the consolidation.
