Mass General Brigham Fax Numbers for Medical Records

One records line, an imaging line per hospital

Mass General Brigham consolidated records requests along with the brand. Its Medical Records page gives one instruction for every hospital: with all required information included, fax or mail the request to Release of Information in Somerville, at (617) 726-3661. The same return block prints on each of the sixteen hospital release forms the page links, from Massachusetts General and Brigham and Women's to Salem, Newton-Wellesley, Spaulding and Wentworth-Douglass. Copies of radiology images and films are the split. Each hospital's release form prints its own contact for images, "For copies of radiology images or films", with a phone number and a fax, and that fax belongs to the hospital that performed the study rather than to Release of Information. The imaging rows above are read from those forms, one per hospital, with the form's page heading as the hospital name. Three Spaulding sites share a line, Salem and Union share a line, and the Spaulding Cambridge form prints no imaging contact at all.

The distinction is between reports and images. A radiology report is text and comes through Release of Information along with the rest of your chart. The images themselves come from the imaging department, sent by electronic transfer by default, with a disc mailed or held for pickup on request. If you need both, expect two requests.

What Mass General Brigham says about how to send the form

The page is explicit about email: "Do not e-mail this request. We cannot guarantee security of all Personally Identifiable Information included in the form if submitted via e-mail." Fax and mail are the two routes it offers for the signed form, alongside requesting records inside the patient portal. It also states that in-person pickup is not available and the records offices remain closed to walk-in service, and that certain information can take up to 30 days to process.

So a fax gets your request in, and the delivery confirmation is your receipt while the request is processed. If you only need the report and not the images, the page directs you to Medical Records rather than the imaging desk.
