One form, both directions

Solis Mammography handles images and reports through a single document, the Request for Release or Transfer of Images and Reports, and it does two jobs. Option 1 retrieves your prior images from another facility so the radiologist reading your new study has something to compare against. Option 2 sends your Solis images and reports somewhere else, to a physician, a hospital, or to you.

The form itself gives the destination: fax to (469) 708-4600 or email to solismammography@solismammo.com. Solis states that a request is fulfilled within 15 days, and asks you to get in touch if nothing has arrived by then.

Solis also uses an electronic image-sharing network for facility-to-facility transfers. If the receiving facility is connected to it, the form has a checkbox for that, and it moves the images without a fax.

One exception is on Solis's own records page: some Solis centers operate as a department of a partner hospital. Solis names Memorial Healthcare System centers specifically and sends those patients to the hospital's own medical-records page instead.

Why a mammogram record is worth chasing

Prior images are not a formality for breast imaging. A radiologist comparing this year's study against last year's is looking for change, and a study read without the prior is read with less information. That is why Option 1 exists on the same form as Option 2, and why the facility that has your old images is the one that has to release them.

What people send Solis:

The release form asks for your name, any previous or maiden name, and your date of birth, so confirm the destination before it leaves:

  • If your Solis center is inside a hospital, check the records page for that hospital instead. Solis says so itself for its Memorial Healthcare System centers.
  • Call the center where you were imaged and ask them to confirm the number before you send anything with your date of birth on it.
  • Skip fax numbers from third-party form sites. Several republish older copies of imaging release forms.
