UPMC routes records requests by hospital

UPMC runs dozens of hospitals across Pennsylvania, New York, and Maryland, and there is no central records office that covers all of them. The instruction UPMC gives, on its own pages and to physician practices, is the same either way: send your request to the hospital where the care happened.

The central Pennsylvania hospitals share one Health Information Department in Harrisburg, so (717) 782-3671 covers UPMC Harrisburg, Memorial, Carlisle, Hanover, Lititz, West Shore, and York between them. UPMC Williamsport runs its own Health Records Department at (570) 321-2138, and UPMC Western Maryland its own at (240) 964-8447.

The Pittsburgh-area hospitals route differently again. UPMC directs physician practices requesting records there to an online portal run with a records vendor rather than to a fax line, and the hospital’s own medical records page carries the current instruction for patients.

What UPMC patients send

Every route starts with the same document, a signed Authorization to Use or Disclose Health Information:

  • Your own copy of a hospital record, sent to you or to a new provider
  • Records moving to a clinic outside UPMC when you change doctors
  • Disability, FMLA, or leave paperwork a UPMC physician has completed
  • Imaging, which several UPMC hospitals route to a separate department from the written chart

UPMC Western Maryland asks for a copy of your photo ID and a phone number with the form, and states that unsigned requests are not processed. That is a good default everywhere: sign it, attach the ID, include a number they can reach you on. An authorization with an ID copy is two or three pages, which is inside the free monthly allowance.

Verify the number before you send

A signed authorization identifies you and names the records being released. Confirm the destination first:

  • Match the fax number to the hospital that treated you. A request faxed to Harrisburg does not reach Williamsport.
  • If UPMC sent you a form or a letter, use the number printed on it. That belongs to the department expecting your document.
  • Not every UPMC hospital will fax records back to you. UPMC Western Maryland states it cannot, and mails the copies or holds them for pickup instead.
  • Requests for records at Pittsburgh-area hospitals may go through a portal rather than a fax. Check that hospital’s own medical records page before assuming fax is available.
