Which Walgreens line you need

Prescription records belong to the Walgreens pharmacy. The Request to Access, Inspect, or Obtain Protected Health Information form prints the destination on the form itself: Walgreens Custodian of Records, 1901 East Voorhees Street, MS 735, Danville, Illinois 61834, fax (217) 554-8955.

Clinic records are separate. Walgreens Healthcare Clinic visits are handled by Take Care Health Services, at fax (888) 297-8357, marked Attention: PSC - ROI Department, with mail going to 8337 South Park Cir., Orlando, FL 32819. Walgreens states these clinics closed at the end of 2019 and that records requests still route through this team.

A store’s own fax number is not either of these. A records request sent to a store is not worked.

What the request form covers

The Walgreens access form lets you name exactly what you want:

• Your entire prescription record, or a date range you specify
• An immunization record, including COVID-19 test results
• Buyout records, meaning prescriptions from a pharmacy Walgreens acquired, where you name the original pharmacy
• Specialty records, which the form handles under its “other” option
• Delivery by mail, by fax to a number you give, or by encrypted email

Walgreens states you should allow up to 15 business days for a response, and that typed signatures are not accepted, so the form has to be signed by hand and scanned or photographed.

Verify the number before you send

The form carries your name, birth date, previous addresses, and the records being released.

• Download the form from walgreens.com and read the destination printed at the top of the copy you downloaded. The form is revised periodically, and the printed version is current.
• Match the form to the record: the pharmacy access form for prescriptions, the Take Care Health Services form for clinic visits.
• Recent prescription history is also in your Walgreens account online, with nothing to sign and nothing to send.
