How to get your Walmart pharmacy records

The routes Walmart publishes:

• Hand the completed form to your local Walmart or Sam’s Club Pharmacy. This is the fastest route, because the store can often pull recent history at the counter.
• Download and print pharmacy history from your Walmart account, which covers recent fills and a payment summary for tax purposes.
• Mail the Request to Access Records form to HIPAA Compliance in Bentonville for periods your account does not cover.
