The office asks for your case number on every page. Put it on every page you send, and keep the fax receipt. An office that cannot match a page to a case has to set it aside, and the receipt is what proves you sent it before the deadline.
Call before you send. Agency fax lines change and some stop answering. A short call confirms the number and whether that office is the one holding your case.
The Health Care Authority lists the income and deduction documents the Apple Health verification line takes, and asks for them within 15 days of the date on the request letter.
The Health Care Authority says documents it receives by email, fax or mail are loaded into the electronic case file and ready to work within three business days, while documents uploaded to Washington Healthplanfinder are visible right away.
DSHS says a signed application is required in some cases, so a Basic Food or cash application sent to the Customer Service Center fax carries the applicant's signature.
The Employment Security Department asks that eligibility forms and documents, including completed job-loss questionnaires, go to its Unemployment Insurance Imaging unit by fax or mail.
Washington Paid Family and Medical Leave asks that the certification form be completed by the applicant and signed by a health care provider before it is faxed or uploaded.
The Department of Revenue asks that a form or letter go to the program address or fax on its mail, fax and teletype list rather than to a general number.
