A West Virginia Medicaid case is worked by the county Department of Human Services office, so a document for a case goes to that office, the one listed in the state directory of county offices.
West Virginia provides a Change Reporting Form, DFA-SNAP-2, with its SNAP notices, and takes it at either the customer service centre or the local county office.

Yes, and that row quotes the agency saying so. Faxing a document to an office that does not accept faxes loses the document with no record that you sent it, so use the channel the row names instead.
