Aetna’s Member Complaint and Appeal Form gives two ways to return it: mail to Aetna, PO Box 14463, Lexington, KY 40512, or “our National Fax Number”, (859) 425-3379. The same form is used for medical and dental plans and for both pre-service and post-service appeals.

One thing to check before sending: if you are appealing on someone else’s behalf, including a spouse or an adult child, Aetna asks for a completed Authorized Representative Form with the appeal. Sending the appeal without it is the most common reason a member appeal comes back unworked.

What to include with an Aetna appeal

The form asks for the details that let Aetna locate the decision you are contesting:

• The member ID number and group number from the front of the ID card, plus plan type
• Whether the appeal is pre-service, meaning a denied authorization, or post-service, meaning a denied claim
• The claim ID for a post-service appeal, or the reference number for a pre-service one
• The date of service, or the date of the denial
• An explanation of the request, with bills, correspondence, and any letter from your doctor attached

Aetna states you do not need to send Explanation of Benefits statements, since it has those already. A form plus a doctor’s letter is typically three to six pages.
