Empower Fax Number for Beneficiary Claims and Plan Forms

The beneficiary-claim fax line is gone

Empower published a fax number for beneficiary notification and death claims through at least August 2026. As of this page’s last check, its beneficiary-claim page lists none: only a phone line, 866-442-3888, and an online form that starts the claim. Empower’s general contact page has never listed a fax number either.

This page will not print the retired number. A death certificate, letters testamentary, or trust documentation for an estate claim is one-shot paperwork. Sending it to a line Empower no longer monitors is worse than a stale number, because there is no confirmation and no way to know it needs re-sending until the claim stalls.

Everything else on Empower’s side routes the way it always has. Participant distribution, rollover and hardship forms still say to return them to the service center at the fax number listed on the form, because service centers are assigned per plan. There is no single participant-forms fax to publish, and this page does not invent one.

Two name changes still send people here. Empower acquired Prudential’s retirement business in 2022 and MassMutual’s retirement plan business before that, so a search for a Prudential retirement fax number or a MassMutual retirement fax number can land on Empower. If your old statement carried one of those names and your new one says Empower, this is the right place for the retirement-plan side of that search.

How to file a beneficiary claim now

Empower’s beneficiary-claim page is the current path: start the claim online with its "Notify us" form, or call 866-442-3888 (weekdays, 7 a.m. to 5 p.m. MT) and a representative takes the death certificate and any estate documents from there.

If a representative gives you a fax number for your specific claim, treat it the same as any number that does not appear on a page you can read yourself: write down who gave it to you and when, and read it back to confirm before you send anything to it.

What still goes to Empower by fax

Participant forms are the fax that survived:

• Distribution requests
• Rollover paperwork
• Hardship withdrawal forms
• Qualified domestic relations orders

Each one carries the fax number for the service center that handles your plan, printed on the form itself. Like the old beneficiary line, these are intake-only: a service representative works the request, and Empower responds by mail or by phone to the contact details on the form. Nothing comes back by fax and there is no fax acknowledgment.

Verify the number before you send

• Read the fax number off your own form. Participant forms name a plan-specific service center, and that number is the correct destination for that form.
• Check the servicing company on your most recent statement. Plans that used to say Prudential or MassMutual now say Empower, and the routing follows the current name.
• Photocopy a death certificate and check the copy before faxing it anywhere. Certificates are printed on security paper that often reproduces badly, and a copy that is hard to read will fax badly too.
• Never fax wire instructions, a full bank routing and account pair, or login credentials. A sent fax cannot be recalled.
