Everything else routes differently. Empower’s participant distribution, rollover and hardship forms say to return them to the service center at the fax number listed on the form, because service centers are assigned per plan. There is no single participant-forms fax to publish, and this page does not invent one.

Two name changes send people here. Empower acquired Prudential’s retirement business in 2022 and MassMutual’s retirement plan business before that, so a search for a Prudential retirement fax number or a MassMutual retirement fax number now lands on Empower. What people fax to Empower

The beneficiary line takes the paperwork that follows a death:
