Empower Fax Number for Beneficiary Claims

One published line, and it is the beneficiary claim line

Empower’s contact page lists no fax number. The one Empower does publish sits on its beneficiary-claim page: (866) 633-5212, for the beneficiary notification form and the death-claim package that goes with it.

Everything else routes differently. Empower’s participant distribution, rollover and hardship forms say to return them to the service center at the fax number listed on the form, because service centers are assigned per plan. There is no single participant-forms fax to publish, and this page does not invent one.

Two name changes send people here. Empower acquired Prudential’s retirement business in 2022 and MassMutual’s retirement plan business before that, so a search for a Prudential retirement fax number or a MassMutual retirement fax number now lands on Empower. If your old statement carries one of those names and your new one says Empower, this is the right place.

What people fax to Empower

The beneficiary line takes the paperwork that follows a death:

• The beneficiary notification form
• A certified death certificate
• Letters testamentary or letters of administration where an estate is involved
• Trust documentation when a trust is the named beneficiary

Empower gives one practical instruction for this on its own page: photocopy a death certificate before faxing it and look at the copy, because certificates are printed on security paper that often reproduces badly. If the copy is hard to read, the fax will be too.

Participant forms, meaning distributions, rollovers, hardship withdrawals and qualified domestic relations orders, go to the fax number printed on the form itself.

The line takes documents in, it does not answer

The beneficiary claim fax is an intake line. Your package arrives, a claims processor works it, and Empower responds by mail or by phone to the contact details on the form. Nothing comes back by fax and there is no fax acknowledgment.

Verify the number before you send

• Read the fax number off your own form. Participant forms name a plan-specific service center, and that number is the correct destination for that form.
• Check the servicing company on your most recent statement. Plans that used to say Prudential or MassMutual now say Empower, and the routing follows the current name.
• Photocopy a death certificate and check the copy before faxing, as Empower advises on its beneficiary page.
• Never fax wire instructions, a full bank routing and account pair, or login credentials. A sent fax cannot be recalled.
