Voya Fax Number for Retirement Plan Forms

The number is on the form, not on the contact page

Voya’s main contact page lists no fax number. The number people are looking for is printed on the plan forms themselves: (800) 643-8143, the forms fax for Voya Retirement Insurance and Annuity Company, alongside the Hartford mailing address.

That is the general VRIAC forms line, and it is the right destination for most workplace retirement plan paperwork. But many Voya plan forms say to fax to the number shown on the form, because large plans get their own service center. If your form names a different number, use the one on your form. It is more specific and it is current for your plan.

What people fax to Voya

The forms that name a fax destination are the plan-level ones:

• Incoming rollover forms, with the prior plan’s statement attached
• Required minimum distribution requests
• Distribution and withdrawal request forms signed by the plan administrator
• Beneficiary designation and maintenance forms
• Direct deposit authorization for benefit payments

Voya’s forms fax is an intake line. Pages arrive at the processing center, a person works the queue in order, and Voya responds by mail, by phone, or in your plan account. Nothing comes back by fax and there is no fax acknowledgment.
