Which MetLife line you need

MetLife has no general customer-service fax. Each claim type has its own processing unit with its own number, and MetLife publishes them in different places: the group life claims fax appears in the FAQ on its file-a-claim page, and the disability and annuity numbers are in the forms library next to the forms they belong to.

Group life claims, meaning a death claim on life insurance held through an employer, go to (570) 558-8645 in Scranton. Disability claims, including the attending physician statement and leave certifications, go to (800) 230-9531 in Lexington. Employer-purchased annuity and 403(b) service forms go to (877) 549-5834 in Des Moines.

One redirection is worth knowing. MetLife spun off its retail annuity and individual life business as Brighthouse Financial in 2017, so a policy bought directly rather than through an employer is likely serviced by Brighthouse now. The Brighthouse guide covers that line.
